四君子汤加味治疗对小儿呼吸机相关性重症肺炎患儿肺功能及炎症指标的影响

摘　　要：目的探讨四君子汤加味治疗对小儿呼吸机相关性重症肺炎患儿肺功能及炎症指标的影响。方法将60例呼吸机相关性肺炎患儿随机分为2组各30例。对照组采取常规综合治疗,观察组在对照组的基础上加用四君子汤加味治疗。比较2组患儿治疗前后的肺功能、炎症指标和疗效。结果治疗后,2组患儿的肺活量(VC)、第1秒用力呼气容积(FEV1)和最大自主分钟通气量(MVV)指数均显著提高(P<0.05),且观察组各项指标显著高于对照组(P<0.05)。2组患者的各项炎症指标显著降低,且观察组红细胞沉降率、降钙素原水平显著低于对照组(P<0.05)。观察组总有效率90.00%,显著高于对照组56.67%(P<0.05)。结论四君子汤加味治疗小儿呼吸机相关性重症肺炎效果显著,能改善患儿肺功能,降低炎症指标水平,提高治愈率。Objective To investigate the effect of modified Sijunzi Decoction on pulmonary function and inflammatory indexes in children with severe ventilator-associated pneumonia.Methods A total of 60 children with ventilator-associated pneumonia treated in our hospital were randomly were divided into control group and observation group according to the random number table method,with 30 cases in each group.The control group was treated with routine comprehensive treatment,while the observation group was treated with modified Sijunzi Decoction on the basis of the control group.After 60 days of treatment,the pulmonary function,inflammation index and treatment efficiency of the two groups were compared before and after treatment.Results After treatment,the vital capacity(VC),forced expiratory volume(FEV 1)and maximum autonomous minute ventilation index(MVV)of the two groups were significantly increased(P<0.05),and the observation group was significantly higher than the control group(P<0.05).The inflammatory indexes of the two groups were significantly decreased,and the Erythrocyte sedimentation rate(ESR)and procalcitonin(PCT)level of the observation group were significantly lower than that of the control group(P<0.05).The effective rate of the observation group was significantly higher than that of the control group(90.00%vs.56.67%,P<0.05).Conclusion Modified Sijunzi Decoction has significant efficacy in the treatment of severe ventilator-associated pneumonia in children,which can improve lung function,reduce inflammatory index and improve the cure rate of severe ventilator-associated pneumonia in children.
