万古霉素骨水泥及颗粒骨治疗小腿创伤后骨髓炎伴骨及软组织缺损的临床效果观察

摘　　要：目的:观察万古霉素骨水泥及颗粒骨治疗小腿创伤后骨髓炎伴骨及软组织缺损的临床效果。方法:选取30例小腿创伤后骨髓炎伴骨及软组织缺损患者为研究对象,按照就诊单双号将其分为对照组和观察组各15例。观察组接受万古霉素骨水泥填充及颗粒骨植骨联合带蒂组织瓣修复术治疗,对照组接受普通骨水泥填充及颗粒骨植骨联合带蒂组织瓣修复术治疗。比较两组骨折愈合率、骨折愈合时间、创面愈合时间和骨髓炎窦道愈合时间。结果:观察组患者骨折愈合率为80.00%(12/15),与对照组的60.00%(9/15)相比,差异有统计学意义(P<0.05);观察组创面愈合时间、骨折愈合时间、骨髓炎窦道愈合时间均短于对照组,差异具有统计学意义(P<0.05)。结论:采用万古霉素骨水泥及颗粒骨联合带蒂组织瓣修复术治疗小腿创伤后骨髓炎伴骨及软组织缺损的效果可靠,可促进患者快速康复。Objective:To investigate clinical effects of Vancomycin bone cement and granular bone in treatment of osteoporosis with bone and soft tissue defects after leg trauma.Methods:30 cases with osteoporosis with bone and soft tissue defects after leg trauma were selected as the study subjects.According to even and odd numbers,they were divided into the control group(n=15)and the observation group(n=15).The observation group received Vancomycin bone cement filling,granule bone graft and pedicle tissue flap repair,while the control group received general bone cement filling,granule bone graft and pedicle tissue flap repair.The fracture healing rate,fracture healing time,wound healing time and osteomyelitis sinus healing time were compared between the two groups.Results:The fracture healing rate of the observation group(80.00%,12/15)was higher than that of the control group(60.00%,9/15),and the difference was statistically significant.The wound healing time,fracture healing time and osteomyelitis sinus healing time of observation group were shorter than those of the control group,and the differences were statistically significant(P<0.05).Conclusions:For the treatment of osteoporosis with bone and soft tissue defects after leg trauma,Vancomycin bone cement filling,granule bone graft and pedicle tissue flap repair is effective and safe,and can promote rapid recovery of the patients.
