摘  要:目的从基因水平了解冀南地区慢性丙型肝炎与HLA-B位点等位基因频率的相关性,初步探索慢性丙型肝炎的保护性基因和易感性基因,为慢性丙型肝炎治疗策略的制定、预后的评估及疫苗的研制提供科学依据。方法应用PCR-序列特异性引物技术(PCR-sequence primers,PCR-SSP)进行HLA-B等位基因分型测定,采用病例对照研究,比较冀南地区慢性丙型肝炎患者219例与健康对照者212例之间HLA-B等位基因频率分布的差异,进一步采用卡方检验分析HLA-B等位基因频率在HCV感染后自发清除组和持续感染组之间是否有差异,从而获得丙型肝炎病毒感染与HLA-B基因多态性的关联性。结果冀南地区丙型肝炎患者的HLA-B*15/35/55等位基因频率明显高于健康对照组(P <0. 05);丙型肝炎患者的HLA-B*40/58等位基因频率明显低于健康对照组(P <0. 05),其他各等位基因在两组间的表达频率差异均无统计学意义; HCV感染后自发清除组患者的HLA-B*15/40等位基因频率明显高于HCV持续感染组患者(P <0. 05),HCV感染后自发清除组患者的HLA-B*55等位基因频率明显低于HCV持续感染组患者(P <0. 05);其他各等位基因在两组间的表达频率差异均无统计学意义。结论 HLA-B*15/35/55等位基因可能为HCV感染的易感基因,HLA-B*40/58等位基因可能为HCV感染的保护性基因;携带HLA-B15/40等位基因的人群感染HCV后容易发生自发清除,而携带HLA-B*55等位基因的人群可能更易转为慢性丙型肝炎。Objective To investigate the relationship between HLA-B allele frequencies and chronic hepatitis C in southern Hebei province,and to explore the protective and susceptible genes for chronic hepatitis C,so as to provide scientific basis for the formulation of treatment strategies,prognosis evaluation and vaccine development.Methods HLA-B allele frequencies were determined by PCR-sequence primers.A case-control study was conducted to compare the HLA-B allele frequencies between 219 chronic hepatitis C patients and 212 healthy controls in southern Hebei province.Chi-square test was used to analyze the difference of HLA-B allele frequencies between spontaneous clearance group and persistent infection group for obtaining the association between hepatitis C virus infection and HLA-B gene polymorphism.Results The distribution of HLA-B alleles in patients with hepatitis C in southern Hebei province were basically the same as those in healthy controls,but the HLA-B*15/35/55 allele frequencies of the patients with hepatitis C were significantly higher than those of the healthy controls(P<0.05).The frequency of HLA-B*40/58 allele in patients with hepatitis C was significantly lower than that in healthy controls(P<0.05).There was no significant difference in the distribution of other alleles between the two groups.The frequency of HLA-B*15/40 allele in patients with spontaneous clearance of HCV infection was significantly higher than that in patients with persistent HCV infection(P<0.05).The allele frequency of HLA-B*55 in spontaneous clearance group was significantly lower than that in HCV persistent infection group(P<0.05),but there was no significant difference in the distribution of other alleles between the two groups.Conclusion HLA-B*15/35/55 allele may be a susceptible gene for HCV infection,and HLA-B*40/58 allele may be a protective gene for HCV infection.Populations with HLA-B15/40 allele are prone to spontaneous clearance of HCV infection,while those with HLA-B*55 allele may be more susceptible to chronic hepatitis
