摘  要:目的观察尼可地尔对糖尿病心肌病(DCM)模型大鼠心脏结构及功能的影响。方法以健康的SD雄性大鼠8只作为正常对照组(C组),喂养普通饲料。另取健康的SD大鼠32只作为实验组,并采用高糖高脂饲料喂养,同时以腹腔注射链脲佐菌素(STZ)的方法建立DCM大鼠模型,将实验组大鼠随机分为糖尿病心肌病模型组(DCM组)、尼可地尔干预组(N组)、格列本脲(K通道拮抗剂)干预组(G组)、尼可地尔+格列本脲干预组(N+G组),每组8只,喂养至第8周后分别予不同的干预。模型组干预措施为连续28 d生理盐水4 ml/kg 1次/d灌胃,N组为连续28 d尼可地尔10 mg/kg 1次/d灌胃,G组为连续28 d格列本脲5 mg/kg 1次/d灌胃,N+G组为连续28 d尼可地尔10 mg/kg+格列本脲5 mg/kg 1次/d灌胃。所有大鼠第12周末行超声心动图测定,包括心脏舒张末室间隔厚度(IVSd)、收缩末室间隔厚度(IVSs)、左室舒张末内径(LVIDd)、左室收缩末内径(LVIDs)、左室舒张末后壁厚度(LVPWd)、左室收缩末后壁厚度(LVPWs)、射血分数(EF)、短轴缩短率(FS)。结果与正常对照组相比,DCM组、N组、G组、N+G组射血分数(EF)下降、短轴缩短率均减小(P <0. 05),IVSd、IVSs、LVP-Wd、LVPWs、LVIDd、LVIDs均增大(P <0. 05)。与DCM组相比,N组、G组、N+G组射血分数、短轴缩短率均增大(P <0. 05),IVSd、IVSs、LVPWd、LVPWs、LVIDd、LVIDs各项指标也均有减小,但以N组减小程度更显著(P <0. 05)。结论尼可地尔在一定程度上可缓解糖尿病心肌病大鼠的心室重构及心功能减退。Objective To explore the effect of nicorandil on the heart structure and function of diabetic cardiomyopathy(DCM)rats.Methods Eight healthy SD male rats were used as normal control group and fed with common diet.Another 32 healthy SD rats were selected as experimental group and fed with high-sugar and high-fat diet.Meanwhile,DCM rat models were established by intraperitoneal injection with STZ.The rats in experimental group were randomly divided into diabetic cardiomyopathy model group(DCM group),nicorandil intervention group(N group),glyburide intervention group(G group),nicorandil+glyburide intervention group(N+G group).There was eight rats in each group,and all rats received different interventions after feeding for eight weeks.The rats were given normal saline 4 ml/(kg·d)in control group,nicorandil 10 mg/(kg·d)in N group,glyburide 5 mg/(kg·d)in G group,and nicorandil 10 mg/(kg·d)+glyburide 5 mg/(kg·d)in N+G group.The rats in experimental group were treated for 28 d.All rats underwent echocardiography at the end of 12th week,including end-diastolic ventricular septal thickness(IVSd),end-systolic ventricular septal thickness(IVSs),end-diastolic left ventricular inner diameter(LVIDd),end-systolic left ventricular inner diameter(LVIDs),end-diastolic left ventricular wall thickness(LVPWd),end-systolic left ventricular wall thickness(LVPWs),ejection fraction(EF),and short axis shortening rate(FS).Results Compared with control group,the cardiac function was decreased in DCM group.Compared with control group,EF and short axis shortening rate decreased in DCM group,N group,G group,and N+G group(P<0.05),while IVSd,IVSs,LVPWd,LVPWs,LVIDd,LVIDs increased(P<0.05).Compared with DCM group,EF and short axis shortening rate increased in N group,G group,and N+G group(P<0.05),while IVSd,IVSs,LVPWd,LVPWs,LVIDd,LVIDs decreased(P<0.05),especially in N group.Conclusion Nicorandil can relieve ventricular remodeling and cardiac dysfunction in diabetic cardiomyopathy rats to a certain extent.
