对埋头加压螺钉结合线缆固定治疗髌骨骨折的疗效分析

摘　　要：目的探讨埋头加压螺钉结合线缆治疗简单横行髌骨骨折的治疗效果。方法回顾分析2015年7月至2016年7月北京积水潭医院创伤骨科收治的15例患者,均为摔伤导致髌骨骨折,其中男10例,女5例;年龄18～55(36.2±9.6)岁;左侧7例,右侧8例。所有患者均行切开复位埋头加压螺钉结合线缆固定术。术后18个月评价患者膝关节主动活动度,并采用Bostman膝关节评分标准评定功能。结果 15例患者手术时间45～70(55.9±8.5)min,随访时间18～24(19.8±3.5)个月;在术后3个月内所有骨折获得骨性愈合。膝关节最终活动度为屈曲平均110(100～140)°,伸直平均5(0～10)°,膝关节活动范围105(95～140)°。Bostman评分为25～30(28.2±1.8)分;无患者出现切口愈合不良,钛缆断裂或内固定失效。结论埋头加压螺钉结合线缆治疗简单横行髌骨中部骨折可以获得满意的治疗效果。Objective To evaluate the results of headless compression screws and cable system for simple transverse patellar fractures.Methods A total of 15 patients with patellar fractures were treated by headless compression screws and cable system in Department of Orthopedic Trauma,Beijing Jishuitan Hospital,from July 2015 to July 2016.They were 10 males and 5 females with the age of(36.2±9.6)years.The fractures affected 7 left and 8 right sides.Active movement and function of the knee joint was evaluated by the Bostman system at 18 months postoperatively.Results All the 15 patients were followed up for an average of 19.8(18-24)months.The average duration of operation was 55.9(45-70)min.All fractures healed within 3 months.The final flexion of knee joint averaged 110(100-140)°,extension averaged 5(0-10)°,the range of motion of knee averaged 105(95-140)°.The mean score of Bostman system was 28.2(25-30)scores.No healing problem of incision,infection or failure of hardware was found.Conclusion Headless compression screws and cable systems can achieve satisfactory outcomes for simple transverse patellar fractures.
