摘  要:目的对B超脐血流测定与胎心监护在诊断胎儿宫内窘迫中的应用价值进行分析。方法 100例孕妇,利用多普勒胎儿脐动脉血流监测仪测定胎儿脐血流,同时利用胎心监护仪监护胎心,根据胎儿脐血流测定结果及胎心监护结果不同分为Ⅰ组(B超脐血流测定与胎心监护均正常, 72例)、Ⅱ组(脐血流测定正常、胎心监护异常, 4例)、Ⅲ组(脐血流测定异常、胎心监护正常, 20例)及Ⅳ组(B超脐血流测定与胎心监护均异常, 4例)。比较四组新生儿Apgar评分、羊水污染程度及胎心异常、变异减速(VD)及晚期减速(LD)、缩宫素激惹实验(OCT)(+)/宫缩压力实验(CST)(+)发生情况。结果 (1)Ⅰ组新生儿1 min Apgar评分≤7分发生率为0,低于Ⅱ组、Ⅲ组、Ⅳ组的25.0%、40.0%、25.0%,差异具有统计学意义(P<0.05)。(2)四组孕妇羊水污染程度比较差异无统计学意义(P>0.05)。(3)Ⅱ组、Ⅳ组胎儿胎心异常发生率高于其他两组,差异具有统计学意义(P<0.05)。(4)Ⅱ组及Ⅳ组胎儿OCT(+)/CST(+)发生率高于Ⅰ组、Ⅲ组,差异具有统计学意义(P<0.05);Ⅳ组胎儿VD及LD发生率高于其他三组,差异具有统计学意义(P<0.05)。结论 B超脐血流测定与胎心监护在诊断胎儿宫内窘迫中具有重要的应用价值,将两种诊断方式联合一起更保证了检测的准确性,值得推广使用。Objective To analyze the application value of B-ultrasound umbilical blood flow measurement and fetal heart rate monitoring in diagnosis of fetal intrauterine distress. Methods In 100 pregnant women, fetal umbilical artery blood flow was measured by Doppler fetal umbilical artery blood flow monitor, and fetal heart rate was monitored by fetal heart rate monitor. According to the results of fetal umbilical blood flow measurement and fetal heart rate monitoring, the patients were divided into group Ⅰ(normal B-ultrasound umbilical blood flow measurement and fetal heart rate monitoring, 72 cases), group Ⅱ(normal umbilical blood flow measurement and abnormal fetal heart rate monitoring, 4 cases), group Ⅲ(abnormal umbilical blood flow measurement and normal fetal heart rate monitoring, 20 cases) and group Ⅳ(abnormal B-ultrasound umbilical blood flow measurement and fetal heart rate monitoring, 4 cases). Comparison were made on Apgar score, amniotic fluid contamination and fetal heart abnormality, variability deceleration(VD) and late deceleration(LD), oxytocin challenge test(OCT)(+)/contraction stress test(CST)(+) in four groups. Results(1)Group Ⅰ had incidence of 1 min Apgar score <7 points as 0, which was lower than 25.0%, 40.0% and 25.0% in group Ⅱ,Ⅲ and Ⅳ, and their difference was statistically significant(P<0.05).(2) There was no significant difference in amniotic fluid pollution among four groups(P>0.05).(3) Group Ⅱ and Ⅳ had higher incidence of abnormal fetal heart rate than that in other two groups, and the difference was statistically significant(P<0.05).(4) Group Ⅱ and Ⅳ had higher incidence of OCT(+)/CST(+) than group Ⅰ and Ⅲ, and the difference was statistically significant(P<0.05). Group Ⅳ had higher incidence of VD and LD than the other three groups, and the difference was statistically significant(P<0.05).
