儿童肺炎支原体肺炎50例临床特征与高凝状态研究

摘　　要：目的探讨儿童肺炎支原体肺炎(MPP)的临床特征和血液高凝状态。方法随机选择50例在2017年6月1日～12月31日笔者医院住院的肺炎支原体肺炎患儿,非MPP组为同期笔者医院儿童外科行斜疝手术的患儿30例,搜集临床资料和血液高凝状态相关指标。结果 50例MPP患儿男性23例,女性27例,男性与女性之比为0. 85∶1,平均年龄5. 22±2. 65岁(最大14岁,最小1岁)。所有患儿均有咳嗽,发热48例、头痛1例、胸痛2例、胸腔积液5例、大叶性肺炎15例、肺不张3例。MPP组的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、D二聚体(DD)、纤维蛋白原(Fib)明显高于非MPP组,差异有统计学意义(P <0. 05)。结论MPP主要发生于学龄前及学龄期儿童,临床特征主要为发热、咳嗽,血液存在高凝状态,是否引起血栓性疾病尚不明确。Objective To explore the hypercoagulable state of children with mycoplasma pneumoniae pneumonia.Methods A randomized study was performed in 50 children with mycoplasma pneumoniae pneumonia at the author′s hospital from June 1 st to December 31 st 2017.Non-MPP group were collected from subjects that presented to oblique hernia surgery at the same time.The datum included clinical features and indicators of hypercoagulable state.Results Among the 50 children,23 were male and 27 were female.The average age was 5.22±2.65 years old.cough and fever were the main clinical manifestations.All of the patients had cough and 48 children had fever.One case had headache and two cases had chest pain.Five children had pleural effusion.Fifteen children were lobar pneumonia and three pulmonary atelectasis.The PT,APTT and the expressions of D-Dimer and fibrinogen in MPP group were all significantly higher than non-MPP group(P<0.05).Conclusion Mycoplasma pneumoniae pneumonia(MPP)mainly happens in the preschool children and school-age children.Most of patients have fever and cough.They have hypercoagulation state,but what cause thrombotic disease is not clear.
