糖尿病手术患者麻醉方式对术后感染与血流动力学及血糖的影响

摘　　要：目的分析糖尿病手术患者麻醉方式对术后感染与血流动力学及血糖的影响。方法该院2014年8月—2017年11月期间收治的采取手术治疗患者中,伴随糖尿病的120例自愿加入该次实验研究,纳入实验后将120例分为实验组和对照组,实验组患者采取Jorg Bubeck硬膜外麻醉,对照组采用常规硬膜外麻醉,对比两组患者的术后感染率和血流动力学以及血糖改善情况。结果实验组患者手术后的各个时间点出现感染的几率明显低于对照组,差异有统计学意义(P<0.05)。实验组患者HR、MAP、SBP、DBP等血流动力学指标在各个时间点的水平相比去对照组更加稳定,差异有统计学意义(P<0.05)。实验组和对照组患者在各个时间点的血糖指标对比差异无统计学意义(P>0.05)。结论为伴随糖尿病的患者进行手术治疗的同时,采取的麻醉方式会对患者术后感染率和血流动力学产生极大的影响,患者的血糖水平无较大改善,采取Jorg Bubeck硬膜外麻醉处理,有利于降低患者术后感染率,促进血流动力学水平的稳定。Objective To analyze the effects of anesthesia methods on postoperative infection,hemodynamics and blood glucose in patients undergoing diabetes surgery.Methods 120 patients with diabetes who were admitted to our hospital from August 2014 to November 2017 were enrolled in this experimental study.After the experiment,120 patients were divided into experimental group and control group.Patients in the experimental group underwent Jorg Bubeck epidural anesthesia,and the control group received conventional epidural anesthesia to compare postoperative infection rates and hemodynamics with improved blood glucose.Results The incidence of infection at each time point after surgery was significantly lower in the experimental group than in the control group(P<0.05).The hemodynamic parameters such as HR,MAP,SBP and DBP in the experimental group were more stable at each time point than in the control group,being statistically significant(P<0.05).There was no significant difference in blood glucose index between the experimental group and the control group at each time point,indicating that it was not statistically significant(P>0.05).Conclusion At the same time as surgery for patients with diabetes,the anesthesia method will have a great impact on the postoperative infection rate and hemodynamics.The patient's blood glucose level is not greatly improved.Jorg Bubeck is used for epidural.Anesthesia treatment is beneficial to reduce the postoperative infection rate and promote the stability of hemodynamics.
