腰围、腰高比对2型糖尿病预测价值的比较

摘　　要：目的比较腰围(WC)、腰高比(WHtR)与2型糖尿病相关性的差异及其对于2型糖尿病的预测价值。方法选取2016年1—6月于该院门诊体检人员327名(年龄36～69岁),分别进行询问病史、体格检查(测量身高、体重、腰围等)、检测空腹静脉血糖等生化指标,计算WHtR。受试者中依据1999WHO糖尿病诊断标准诊断的2型糖尿病患者116名,对糖尿病患者与非糖尿病患者的WC、WHtR数据进行比较,采用SPSS 18.0统计学软件绘制WC、WHtR预测2型糖尿病的受试者工作特征(ROC)曲线,计算曲线下面积(AUC),并采用MedCalc16.8软件进行比较分析。结果327名受试者中男性122名,2型糖尿病患者41名;女性205名,2型糖尿病患者75名。不同性别间糖尿病患病率无显著差异(P>0.05)。糖尿病患者WC、WHtR均值明显高于非糖尿病患者,差异有统计学意义(P<0.05)。男性WC、WHtR预测2型糖尿的AUC分别为0.63、0.61,二者比较差异无统计学意义(P>0.05);女性WC、WHtR预测2型糖尿病的AUC分别为0.61、0.60,二者比较无统计学差异(P>0.05)。WC预测2型糖尿病的切点,男性92 cm,女性89 cm;WHtR预测2型糖尿病的切点,男性0.54,女性0.57。结论 WC及WHtR均与2型糖尿病密切相关,二者对2型糖尿病的预测价值相似。WC测量简单,无需计算,且敏感性高,临床应用更为简便。Objective To compare the differences between waist circumference(WC),waist height ratio(WHtR)and type 2 diabetes and its predictive value for type 2 diabetes.Methods A total of 327 outpatients(aged 36-69 years old)in our hospital from January 2016 to June 2016 were selected to conduct medical history,physical examination(measuring height,weight,waist circumference,etc.),biochemical indicators such as fasting venous blood glucose,and calculate WHtR.Among the 116 patients with type 2 diabetes diagnosed according to the 1999WHO Diabetes Diagnostic Criteria,the WC and WHtR data of diabetic patients and non-diabetic patients were compared,and the WC and WHtR were used to predict type 2 diabetes by SPSS18.0 statistical software.The work characteristic(ROC)curve,the area under the curve(AUC)was calculated,and comparative analysis was performed using MedCalc16.8 software.Results Of the 327 subjects,122 were men,41 were type 2 diabetes,205 were women,and 75 were type 2 diabetes.There was no significant difference in the prevalence of diabetes between different genders(P>0.05).The mean values of WC and WHtR in diabetic patients were significantly higher than those in non-diabetic patients,and the difference was statistically significant(P<0.05).The AUC of male WC and WHtR predicted type 2 diabetes were 0.63 and 0.61,respectively.There was no significant difference between them(P>0.05).The WC and WHtR predicted the AUC of type 2 diabetes were 0.61 and 0.60,respectively with significant differences(P>0.05).WC predicts the cut-off point of type 2 diabetes,male 92cm,female 89cm;WHtR predicts the cut-off point of type 2 diabetes,male 0.54,female 0.57.Conclusion Both WC and WHtR are closely related to type 2 diabetes,and their predictive value for type 2 diabetes is similar.WC measurement is simple,with no need of calculation,and with high sensitivity and simple clinical application.
