摘 要:目的探讨精索静脉曲张(varicocele,VC)患者睾丸动脉血流动力学参数与精液质量的相关性。方法选取2014年10月—2017年8月空军特色医学中心(原空军航空医学研究所附属医院)泌尿科就诊的部队官兵左侧VC患者73例,其中亚临床型22例,临床Ⅰ级17例,Ⅱ级20例,Ⅲ级14例。通过彩超检测睾丸动脉的阻力指数(resistance index,RI)和搏动指数(pulsatility index,PI)并行常规精液检查。采取方差分析比较不同组别VC患者RI、PI及精子密度、精子活率,进一步组间比较采用Bonferroni检验;采取Pearson相关性分析法检验RI、PI与精液质量相关性。结果睾丸动脉RI、PI随VC级别的增加而升高,除亚临床型和Ⅰ级组间差异无统计学意义,其他任选2个组间差异均有统计学意义(P<0.05);精子密度、精子活率随VC级别的增加而下降,且任选2个组间差异均有统计学意义(P<0.01);RI、PI与精子密度、精子活率均呈显著负相关(P<0.01)。结论应用彩超检测VC患者睾丸动脉的RI、PI能客观评价睾丸的血供情况,同时为预测精子质量提供一定依据。Objective To investigate the correlations between testicular arterial hemodynamic parameters and semen quality in patients with varicocele (VC). Methods Between October 2014 and August 2017, 73 military patients with left VC were selected at the Urology Department of Air Force General Hospital. Among them, 22 were subclinical, 17 were clinical grade Ⅰ, 20 were grade Ⅱ, and 14 were grade Ⅲ. The resistance index(RI)and pulsation index(PI)of the testis artery were detected by color ultrasound, and regular semen tests were conducted. Variance analysis was conducted to compare RI, PI, sperm density and sperm motility between different groups of VC patients. The Bonferroni test was conducted for further inter-group comparison. Pearson correlation analysis was conducted to explore the relationships between RI, PI of the testis artery and semen quality. Results RI and PI of the testis artery increased with the VC level. There was no statistically significant difference between the subclinical patients and those of grade I, but the difference between the other two optional groups was statistically significant (P<0.05). Sperm density and motility decreased with the increase of the VC level, and the difference between the other two optional groups was statistically significant (P<0.01). RI and PI were negatively correlated with sperm density and motility (P<0.01). Conclusion The use of color ultrasound to detect RI and PI of the testis artery in VC patients can not only evaluate the testicular blood supply accurately, but also provide reference for predicting sperm quality.
