静脉微泵催产素引产的应用研究

摘　　要：目的探讨静脉微泵催产素在妊娠晚期引产中的有效性和安全性。方法选取2016年4月至2017年4月收住我院妊娠晚期单胎头位经球囊扩张宫颈后适合催产素引产的孕妇共312例,采用随机数表法分为对照组(155例)和观察组(157例),观察组给予静脉微泵催产素引产,对照组采用传统静脉滴注催产素引产;评价两组产妇的引产疗效、分娩结局及孕妇和助产士的满意度。结果观察组从泵入催产素到产生有效宫缩的时间、第一产程短于对照组,子宫过度刺激例数少于对照组,观察组孕妇和助产士的满意度高于对照组,差异均具有统计学意义。结论静脉微泵催产素比传统静滴催产素在妊娠晚期引产中更安全、有效,且不受体位、活动的影响,增加孕妇的舒适感和满意度,减少助产士临床工作量,临床上值得推广。Objective To explore the efficacy and safety of intravenous micro-pump oxytocin in induced labor in late pregnancy.Methods A total of 312 pregnant women of singleton pregnancy after balloon dilation of cervix in the third trimester of pregnancy who were eligible for oxytocin induction admitted to our hospital from April 2016 to April 2017 were recruited.The pregnant women were randomly divided into the control group(n=155)and the observation group(n=157)by random number table method.The observation group was given intravenous micro-pump oxytocin for labor induction,and the control group was administered oxytocin through traditional intravenous infusion.The effect of induction,the outcome of delivery and the satisfaction of pregnant women and midwifery of two groups were evaluated.Results The observation group had shorter time period from pumping oxytocin to effective uterine contraction,shorter first stage of labor,fewer cases of uterus over-stimulation,and higher satisfaction of pregnant women and midwifery than the control group.The differences were all statistically significant.Conclusion Intravenous micro-pump oxytocin is safer and more effective than traditional intravenous oxytocin in induction of labor in late pregnancy.It is not affected by the body position and activity,and therefore increases the comfort and satisfaction of pregnant women.It also reduces the workload of midwives,and is worthy of being promoted in clinical practice.
