摘  要:目的:探讨纳米银敷料在体表慢性难愈合伤口中的应用价值。方法:按照随机数字法将2017年6月-2018年6月笔者医院收治的60例体表慢性难愈合伤口患者分对照组与观察组,每组30例。对照组给予常规伤口处置,观察组在常规伤口处置的基础上加用纳米银敷料,均连续治疗4周。比较两组患者的伤口愈合效果与愈合时间、伤口愈合速率、不良反应发生情况,治疗前及治疗后2、4周的疼痛数字评分(Numericratingscales,NRS)、肉芽形态评分及伤口病原菌阳性率。结果:观察组甲级愈合率为90.00%明显高于对照组的76.67%,差异有统计学意义(P<0.05)。观察组伤口愈合时间及愈合速率为[(22.78±6.39)d,(41.42±8.10)%]明显优于对照组的[(32.66±8.48)d,(20.30±5.45)%],差异有统计学意义(P<0.05)。治疗后2周、4周的NRS疼痛评分、肉芽形态评分、伤口病原菌阳性率均明显降低,且观察组明显低于对照组,差异有统计学意义(P<0.05)。观察组不良反应发生率为6.67%,对照组为3.33%,组间比较差异无统计学意义(χ~2=2.380,P=0.412)。结论:纳米银敷料能够明显促进体表慢性难愈合伤口的愈合,缓解疼痛,安全可靠。Objective To discuss the application value of nano silver dressing in chronic hard-healing wounds on body surface. Methods 60 patients with chronic hard-healing wounds which was divided into the control group and the observation group according to random number table in our hospital from June 2017 to June 2018, 30 cases in each group. The patients were given conventional wound management in the control group, and the patients were added nano silver dressing based on conventional wound management in the observation group, all the patients were treated for 4 months. The wound healing effect and healing time,wound area healing speed, adverse reaction was compared between two groups. The numerical rating scale(NRS), granulation morphology score, wound pathogen positive rate was compared between two groups before treatment and 2, 4 weeks after treatment. Results The healing rate of grade A in the observation group was 90.00%, which was significantly higher than that in the control group(76.67%), the difference was statistically significant(P<0.05). The wound healing time and rate in the observation group were [(22.78±6.39)d,(41.42±8.10)%] significantly better than those in the control group [(32.66±8.48)d,(20.30±5.45)%, P <0.05]. The NRS pain score, granulation morphology score and the positive rate of pathogenic bacteria in the wound were significantly lower in the observation group than in the control group at 2 and 4 weeks after treatment(P <0.05). The incidence of adverse reactions in the observation group was 6.67% higher than that in the control group(3.33%), there was no significant difference between the two groups(χ2=2.380,P =0.412). Conclusion Nano-silver dressing could obviously promote the healing of chronic hard-healing wounds on body surfaceand relieve the pain. It is safe and reliable.
