摘 要:目的通过测定妊高征患者叶酸干预前后、血清同型半胱氨酸(HCY)及血清锌浓度是否变化,探讨其与妊高征发病的关系,期望为妊高征的防治提供依据。方法采用西门子(xp)化学发光免疫分析仪检测2017年1—12月包头地区妊高症孕妇49例的血清叶酸及同型半胱氨酸的水平,采用原子光谱法进行血清锌水平检测,妊高症的诊断标准依据《妇产科学》第8版,妊高征组包括轻度妊高征组19例,中度妊高征组17例,重度妊高征13例,正常孕妇组为产科门诊已经建档的正常孕妇组50例。正常对照组为该院同期体检的女性正常人群。3组年龄差异无统计学意义,同时测定各组的叶酸、同型半胱氨酸、锌水平。结果正常孕妇的HCY浓度水平(7.64±3.00)μmol/L低于正常对照组(9.20±2.32)μmol/L,差异有统计学意义(t=5.46,P<0.05),妊高症组的HCY水平(19.47±3.88)μmol/L高于前两组,其差异有统计学意义(t=26.7,P<0.05),健康对照组(10.33±3.12)ng/L与正常孕妇组的血清叶酸水平(9.84±3.00)ng/L无明显变化(t=1.12,P>0.05)。妊高症组的叶酸水平(5.66±1.88)g/L明显低于前两组,差异有统计学意义(t=13.44,P<0.05),正常孕妇组的血清锌水平(13.84±2.11)μmo/L明显低于健康对照组(16.02±3.46)μmo/L,差异有统计学意义(t=7.87,P<0.05),妊高症组血清锌(8.21±3.35)μmo/L明显低于正常孕妇组,差异有统计学意义(t=16.98,P<0.05)。对于正常孕妇组的血清HCY及锌水平,在叶酸的干预前后无明显变化(t=1.07,P>0.05);而妊高症组的血清HCY及锌水平,在叶酸干预前后差异都有显著的变化(t=17.33,P<0.05)。结论妊高症患者的血清叶酸及锌水平都比正常组低,而血清HCY水平增高,口服叶酸能够降低妊高症患者的HCY水平并且血清锌水平也降低,因此妊高症患者补充叶酸时考虑适当地补充锌,以便更好地保证母婴的健康。Objective To determine the relationship between serum homocysteine (HCY) and serum zinc concentration before and after folic acid intervention in patients with pregnancy-induced hypertension,and to explore its relationship with the incidence of pregnancy-induced hypertension,and hope to provide a basis for the prevention and treatment of pregnancy-induced hypertension.Methods Siemens (xp) chemiluminescence immunoassay was used to detect serum folate and homocysteine levels in 49 pregnant women with pregnancy-induced hypertension in Baotou area from January to December 2017.Serum zinc levels were detected by atomic spectrometry.The diagnostic criteria were based on the 8th edition of Obstetrics and Gynecology.The pregnancy-induced hypertension group included 19 patients with mild pregnancy-induced hypertension,17 patients with moderate pregnancy-induced hypertension,13 patients with severe pregnancy-induced hypertension,and the normal pregnant group for obstetrics.50 cases of normal pregnant women who have been filed.The normal medical examination group is the normal female population of the same physical examination in our hospital.There was no statistically significant difference in age between the three groups.The levels of folic acid,homocysteine and zinc in each group were also determined.Results The level of HCY in normal pregnant women (7.64±3.00)μmol/L was lower than that in the normal control group(9.20±2.32)μmol/L.The difference was statistically significant (t=5.46,P<0.05).The HCY level in the pregnancy-induced hypertension group(19.47±3.88)μmol/L was significantly higher than the former two groups(t=26.7,P<0.05),serum folate levels in healthy controls (10.33±3.12)ng/L and normal pregnant women(9.84±3.00)ng/L.There was no significant change in ng/L)(t=1.12,P>0.05).The folic acid level in the pregnancy-induced hypertension group (5.66±1.88)g/L was significantly lower than that in the former two groups (t=13.44,P<0.05),and the serum zinc level in the normal pregnant group (13.84±2.11)μmo/L sign
