摘  要:目的探究子宫内膜异位症(EMs)组织细胞超微结构、细胞凋亡变化及Rac1、Cdc42的表达意义,为子宫内膜异位症的治疗寻找新的靶点。方法选取30例卵巢子宫内膜异位囊肿患者的异位囊肿组织作为EMs异位内膜组,对应的30例增生期子宫内膜组织作为EMs在位内膜组,另选择30例正常增生期子宫内膜组织作为正常组。电镜下观察子宫内膜细胞超微结构,流式细胞仪检测细胞凋亡率,免疫组化Envision法检测子宫内膜组织中Rac1、Cdc42表达情况并进行半定量分析,Western blot定量和实时定量PCR法分别检测子宫内膜组织中Rac1、Cdc42蛋白表达情况和Rac1、Cdc42 mRNA表达情况。结果相对于正常组,EMs异位内膜组和EMs在位内膜组细胞超微结构上均存在不同程度的凋亡异常改变,以细胞核、细胞器形态结构异常最为明显。EMs异位内膜组和EMs在位内膜组细胞凋亡率均明显低于正常组(P均<0.05),EMs异位内膜组和EMs在位内膜组比较差异无统计学意义。免疫组化与Western blot检测结果显示,Rac1、Cdc42相对表达量EMs异位内膜组明显高于EMs在位内膜组(P均<0.05),EMs在位内膜组明显高于正常组(P均<0.05)。实时定量PCR检测结果显示,Rac1、Cdc42 mRNA相对表达量EMs异位内膜组明显高于EMs在位内膜组(P均<0.05),EMs在位内膜组明显高于正常组(P均<0.05)。结论子宫内膜异位症组织细胞超微结构异常,细胞凋亡减少,且组织中存在Rac1、Cdc42大量表达,其信号途径可能参与子宫内膜异位症发生、转移,为子宫内膜异位症的治疗提供了新的靶点。Objective It is to investigate the significance of ultrastructural changes, apoptosis and the expression of Rac1 and Cdc42 in endometriosis (EMs), and to find new targets for the treatment of EMs. Methods The ectopic cysts tissue of 30 patients with ovarian endometriotic cysts were selected as the ectopic endometrial group of EMs, the corresponding 30 endometrial tissues of proliferative phase were used as the eutopic endometrium of EMs, and 30 endometrial tissue of normal hyperplasias were selected as a normal group. The ultrastructure of endometrial cells was observed under electron microscope, the apoptosis rate was detected by flow cytometry, the expression of Rac1 and Cdc42 in endometrial tissues was detected by immunohistochemical Envision method and semi-quantitative analysis was performed. Western blot and real-time quantitative PCR were performed to dectect the expression of Rac1, Cdc42 protein and Rac1 and Cdc42 mRNA in endometrial tissues. Results Compared with the normal group, the ultrastructure of EMs ectopic endometrium group and EMs eutopic endometrium group had different degrees of apoptosis abnormalities, and the nucleus and organelle morphology were the most obvious. The apoptotic rate of EMs ectopic endometrium group and EMs eutopic endometrium group was significantly lower than that of normal group ( P <0.05), but there was no significant difference between EMs ectopic endometrium group and EMs eutopic endometrium group. The results of immunohistochemistry and Western blot showed that the relative expression of Rac1 and Cdc42 was significantly higher in ectopic endometrium group than that in EMs eutopic endometrium group ( P <0.05), the expression in EMs eutopic endometrium group was significantly higher than that in the normal group ( P <0.05). The results of real-time quantitative PCR showed that the relative expression of Rac1 and Cdc42 mRNA in EMs ectopic endometrium group was significantly higher than that in EMs eutopic endometrium group ( P <0.05), and the expression in EMs eutopic end
