摘 要:目的探讨经脑沟入路治疗高血压基底节脑出血的临床疗效及短期预后。方法选取2016年1月~2017年7月在我院行经脑沟入路血肿清除术的58例高血压基底节脑出血患者作为观察组,以同期行常规经脑皮质血肿清除术的58例高血压基底节脑出血患者作为对照组,对两组患者的术后临床疗效、生活质量及预后情况进行比较分析。结果与对照组相比,观察组术中出血量、术后1d GCS评分及住院时间明显减少,血肿清除率明显提高(P<0.05);术后6个月,观察组ADL评分明显高于对照组,两组在ADL损害程度上差异具有统计学意义(P<0.05);观察组术后6个月发生癫痫的患者相比对照组明显减少(P<0.05)。结论经脑沟入路行血肿清除术治疗高血压基底节脑出血可有效降低对脑组织的损伤,对短期内生活质量的提高以及降低癫痫的发生率有着积极的作用。Objective To study the clinical curative effect and short-term prognosis in patients of hypertensive cerebral hemorrhage treated by trans-sulcus approach. Methods Selected 58 patients with hypertensive basal ganglia hemorrhage treated by hematoma aspiration trans-sulcus approach from Jan 2016 to Jul 2017 in our hospital as observation group, and 58 patients with hypertensive basal ganglia hemorrhage treated by conventional hematoma aspiration through pallium at the same period as control group, then compared and analyzed the postoperative clinical curative effect, life quality and prognosis of patients in two groups. Results Compared with control group, the blood loss, GCS score at the first day after surgery, and the length of hospital stay of observation group seriously reduced, the hematoma clearance rate increased significantly (P< 0.05). 6 months after surgery, the ADL scores of observation group was obviously higher than that of control group, and the difference of ADL damage degree between two groups had statistical significance (P< 0.05). The occurrence rate of epilepsy in observation group 6 months after surgery significantly decreased more than that of control group(P< 0.05). Conclusion The hematoma aspiration trans-sulcus approach treated hypertensive basal ganglia hemorrhage can effectively reduce the damage of brain tissue, and plays a positive role to improve the quality of life and reduce the occurrence of epilepsy in the short term.
