摘 要:目的研究与比较不同头孢菌素类药物所致尿毒症患者头孢菌素类脑病发生率。方法 600例尿毒症患者,根据采用的头孢类菌素类药物不同分为A组(一代头孢菌素)、B组(二代头孢菌素)、C组(三代头孢菌素)和D组(四代头孢菌素),每组150例。比较四组患者头孢菌素类脑病发生率,并比较四组头孢菌素类脑病患者停药后缓解时间。结果 D组患者头孢菌素类脑病发生率15.33%高于A组的6.67%、B组的8.00%、C组的7.33%,差异具有统计学意义(P<0.05);A组、B组及C组患者头孢菌素类脑病发生率比较,差异无统计学意义(P>0.05)。B组头孢菌素类脑病患者停药后缓解时间长于A组、C组及D组,差异具有统计学意义(P<0.05);A组、C组及D组头孢菌素类脑病患者停药后缓解时间比较,差异无统计学意义(P>0.05)。结论四代头孢菌素类药物所致尿毒症患者头孢菌素类脑病发生率相对较高,而二代头孢菌素所致头孢菌素类脑病的缓解时间相对较长,应给予充分的重视与干预。Objective To study and compare the incidence of cephalosporin encephalopathy in uremic patients caused by different cephalosporins. Methods A total of 600 uremic patients were divided by different cephalosporins into group A (first-generation cephalosporin), group B (second-generation cephalosporin), group C (third-generation cephalosporin) and group D (fourth-generation cephalosporins), with 150 cases in each group. The incidence of cephalosporins encephalopathy was compared among the four groups, and the remission time of cephalosporins encephalopathy after withdrawal was compared among the four groups. Results Group D had higher incidence of cephalosporins encephalopathy as 15.33% than 6.67% in group A, 8.00% in group B and 7.33% in group C. Their difference was statistically significant (P<0.05). Group A, B and C had no statistically significant in incidence of cephalosporins encephalopathy (P>0.05). Group B had longer remission time after withdrawal than group A, C and D, and their difference was statistically significant (P<0.05). Group A, C and D had no statistically significant difference in remission time after withdrawal (P>0.05). Conclusion The incidence of cephalosporin encephalopathy in uremic patients caused by fourth-generation cephalosporins is relatively high, while the remission time of cephalosporin-induced encephalopathy caused by second-generation cephalosporins is relatively long. So full attention and intervention should be given to it.
