维生素D干预临床前期类风湿关节炎的研究

摘　　要：目的了解临床前期类风湿关节炎(pre-RA)发展为类风湿关节炎(RA)的概率;观察维生素D对于pre-RA合并维生素D缺乏者及不足者干预治疗的疗效。方法 97例pre-RA患者,随机分为观察组(51例)及对照组(46例)。观察组予维生素D_2注射液治疗,对照组予以安慰剂,疗程为18个月。观察2组患者类风湿因子(RF)或抗环瓜氨酸抗体(ACPAs)下降至正常例数、出现关节痛例数、出现关节炎的例数及出现RA例数。结果2组患者抗体滴度下降至正常为0例;观察组有2例关节痛患者而对照组有6例,2组间差异有统计学意义;观察组有1例关节炎患者而对照组有3例,2组之间差异无统计学意义;2组患者均未发展为RA。结论维生素D干预治疗,可以减少pre-RA发生关节痛的概率,对关节炎发生无影响。Objective To investigate the probability of pre-clinical rheumatoid arthritis (pre-RA) developing into rheumatoid arthritis (RA);to observe the effect of vitamin D intervention on pre-RA patients with vitamin D deficiency and insufficiency. Methods Ninty-seven pre-RA patients were randomly divided into observation group (51 cases) and control group (46 cases). Vitamin D2 injection was administered in the observation group and placebo was administered in the control group for 18 months. The incidence of rheumatoid factor (RF) or anti-cycloguarine antibody (ACPAs) decreased to the normal, the number of joint pain symptoms, the number of arthritis cases and the number of RA cases were observed. Results No patients of antibody titers decreased to normal in both groups. Two patients in the observation group had joint pain, and six patients in the control group had joint pain. The difference between the two groups was statistically significant. One patient in the observation group had arthritis, and three patients in the control group had arthritis. The difference between the two groups was not statistically significant. None of the patients in both groups developed RA. Conclusion Vitamin D intervention can reduce the incidence of joint pain in pre-RA and has no effect on the occurrence of arthritis.
