摘 要:目的观察与比较新生儿上、下肢PICC置管效果、不良情况及疼痛刺激程度。方法选取2017年7月至2018年3月本院PICC置管新生儿96例作为研究对象,根据置管部位的不同将其等分为A组(上肢静脉置管组)和B组(下肢静脉置管组)。比较两组的一次操作成功率、出血量、操作时间、留置时间、不良情况发生率及疼痛刺激程度。结果 B组的一次操作成功率高于A组(P <0. 05),出血量小于A组(P <0. 05),操作时间短于A组(P <0. 05),留置时间长于A组(P <0. 05),导管异位不良情况发生率低于A组(P <0. 05),疼痛刺激程度优于A组(P <0. 05)。结论新生儿下肢PICC置管效果优于上肢置管,且不良情况及疼痛刺激程度控制效果也更好,因此下肢PICC置管在新生儿中的应用价值更高。Objective To observe and compare the effect of PICC catheterization, adverse conditions and pain stimulation degrees on the upper and lower limbs of newborns. Methods From July 2017 to March 2018, a total of 96 patients with PICC catheterization in our hospital were selected as the study subjects. They were divided into group A (the upper limb venous catheterization group) and group B (the lower limb venous catheterization group ) according to different catheterization sites. The success rate of one operation, the amount of bleeding, the time of operation, the duration of indwelling, the incidence of adverse events, and the degree of pain stimulation were compared between the two groups. Results The success rate of one operation in group B was higher than that in group A( P <0.05). The bleeding volume of group B was lower than that in group A( P <0.05). The operation time of group B was shorter than that in group A( P <0.05). The indwelling time was longer than that in group A( P <0.05). The incidence of catheter ectopic malformation was lower than that in group A( P <0.05). The pain stimulation degree of group B was better than that of group A( P <0.05). Conclusion The effect of PICC catheterization in the lower limbs of the newborn is better than that of the upper limbs, and the adverse effects and the degree of pain stimulation are also better. Therefore, the application of PICC catheterization in the lower limbs is more valuable in neonates.
