糖尿病合并功能性便秘的危险因素及生物反馈治疗的效果观察

摘　　要：目的分析糖尿病合并功能性便秘的危险因素并观察生物反馈治疗的临床效果。方法对390例糖尿病患者进行调查,评估其便秘发生情况,对导致便秘的因素进行Logistic回归分析;对162例糖尿病合并功能性便秘的患者行生物反馈治疗并分析效果。结果糖尿病合并功能性便秘的发生率为41.54%,严格限制饮食、血糖水平过高及具有周围神经病变是糖尿病患者合并功能性便秘的危险因素(P<0.05);生物反馈治疗糖尿病合并功能性便秘治愈率68.52%,出口型梗阻和慢传导型梗阻两种类型的治愈率分别为82.76%、52.00%(P<0.05);出口型梗阻治疗前后直肠感知阈和直肠最大耐受量比较,差异有统计学意义(P<0.05)。结论糖尿病合并功能性便秘的发病率高,生物反馈治疗糖尿病合并功能性便秘临床效果好,尤其适于出口型梗阻。Objective To analyze the risk factors of diabetes complicated with functional constipation and observe the clinical effect of biofeedback therapy.Methods A total of 390 patients with diabetes were investigated to assess the incidence of constipation.Logistic regression analysis was performed on the factors leading to constipation.Biofeedback therapy was performed on 162 patients with diabetes complicated with functional constipation(FC).Results The incidence of constipation in diabetic patients was 41.54%.Strict restriction of diet,excessive blood glucose level and peripheral neuropathy were risk factors for diabetic patients with FC(P<0.05).The cure rate of biofeedback treatment for diabetes complicated with FC was 68.52%;the cure rate of export-oriented obstruction and slow transmitting obstruction was respectively 82.76% and 52.00%(P<0.05).Before and after the treatment of export-type obstruction,the rectal perception threshold and the maximum rectal tolerance were compared,which were statisticly different(P<0.05).Conclusions The incidence of diabetes complicated with FC is high.The clinical effect of biofeedback therapy for diabetes complicated with functional constipation is good,especially for export-type obstruction.
