摘  要:目的研究基质金属蛋白酶在稽留流产患者绒毛组织中的表达变化,探讨其在稽留流产中的作用及其调控机制。方法选取2015年11月至2016年5月期间就诊于苏州大学附属第一医院计划生育门诊和苏州市立医院北区妇产科的稽留流产患者30例作为实验组,同期健康早孕流产患者30例作为对照组。采用ELISA法检测血浆TGF-β1水平,定量PCR(RT-PCR)检测患者绒毛组织中NF-κB、TGF-β1、TIMP-1和MMP-9的mRNA表达情况,采用蛋白印迹法(Western Blot)检测分析NF-κB、TGF-β1、TIMP-1和MMP-9的蛋白水平。结果实验组血浆中TGF-β1的平均水平低于对照组(P<0.05);实验组绒毛组织中MMP-9的mRNA表达量高于对照组,而NF-κB1、TGF-β1和TIMP-1mRNA表达量均低于对照组(P<0.05);实验组绒毛组织中MMP-9的蛋白表达水平高于对照组,NF-κB p50、TGF-β1和TIMP-1的蛋白表达水平均低于对照组(P<0.05)。结论 NF-κB表达水平的降低导致TGF-β1水平降低,从而影响MMP-9/TIMP-1的平衡,使ECM过度降解,导致胚胎植入失败及胎盘形成障碍,最终发生胚胎停育。Objective To investigate the expression changes of matrix metalloproteinases (MMPs) in the villus tissue of patients with missed abortion, and to explore its role and regulatory mechanism in the missed abortion. Methods 30 patients with missed abortion were selected as the research group, and 30 healthy and abortion women in early pregnancy were selected as the control group from November 2015 to May 2016 in the family planning clinic of the First Affiliated Hospital of Soochow University and the Department of Obstetrics and Gynecology, North District of Suzhou Municipal Hospital. The level of TGF-β1 in plasma was measured by ELISA. MRNA expressions of NF-κB, TGF-β1, TIMP-1 and MMP-9 in villus tissue were detected by RT-PCR,and the protein levels of NF-κB, TGF-β1, TIMP-1 and MMP-9 were detected by Western blotting. Results (1)The average level of TGF-β1 in plasma of the research group was lower than that of the control group ( P <0.05).(2)The mRNA expression levels of NF-κB1, TGF-β1 and TIMP-1 in villus tissue of the research group were lower than those of the control group, and the expression level of MMP-9 mRNA was higher than that of the control group ( P <0.05).(3)The expression level of MMP-9 protein in villus tissue of the research group was higher than that in the control group, and the levels of NF-κB p50, TGF-β1 and TIMP-1 protein were lower than those of the control group( P <0.05). Conclusion The decreased expression level of NF-κB can affect the expression level of TGF-β1, and it may influence the balance of MMP-9/TIMP-1 and lead to the excessive degradation of extracellular matrix, and result in the failure of embryo implantation and impaired placenta formation, eventually cause the termination of embryo development.
