补肾活血法对子宫内膜异位症子宫内膜的影响

摘　　要：目的观察补肾活血中药对子宫内膜异位症(EMs)患者着床期内膜同源盒基因(HOXA10)DNA甲基化的影响。方法肾虚血瘀型EMs不孕患者65例随机分为治疗组和对照组,治疗组35例,对照组30例。另选20例同期因男方因素致不孕就诊的女性为空白组。治疗组口服补肾活血免煎颗粒3个月,对照组、空白组直接进入研究。观察3组分泌中期血清雌二醇(E2)、孕酮(P)含量,检测内膜HOXA10蛋白的含量,甲基化特异聚合酶链反应检测子宫内膜HOXA10DNA甲基化程度。结果治疗组分泌中期血清E2、P明显高于对照组(P<0.05)。治疗组HOXA10蛋白呈强阳性(3+)表达,与对照组比较差异有统计学意义(P<0.05)。治疗组EMs患者内膜发生甲基化占14.4%,与对照组比较差异有统计学意义(P<0.05)。结论补肾活血中药可改善EMs不孕患者血清E2、P激素水平,增加内膜HOXA10蛋白的含量,降低内膜HOXA10DNA甲基化率。Objective To observe the effects on HOXA10,DNA eythylation of implantation in the patients with endometriosis treated with the therapy from tonifying kidney and activating blood circulation.Methods A total of 65 infertility patients with endometriosis of kidney deficiency and blood stagnation were randomized into a treatment group(35 cases)and a control group(30 cases).Additionally,a blank group of 20 cases was set up,in which the infertility was caused by husband′s side factor.In the treatment group,bushen huoxue granules prescribed for oral administration for 3 months.No any intervention was given in the control group and the blank group.The levels of estradiol(E 2)and progesterone(P)in midterm secretion were observed in the three groups.HOXA10 protein level was determined with intimal isogenesis kit.HOXA10 methylation degree was determined with methylation specific polymerase chain reaction.Results The serum E 2 and P in midterm secretion in the treatment group were higher obviously than the control group( P <0.05).HOXA10 protein was strongly positive(3+)in the treatment group,indicating the significant difference as compared with the control group( P <0.05).The endometrial methylation accounted for 14.4% in the treatment group,indicating the significant difference as compared with the control group( P <0.04).Conclusion The Chinese herbal medicine for tonifying kidney and activating blood circulation improves in the levels of serum E 2 and P,increases HOXA10 protein expression and reduces HOXA10 methylation rate.
