正畸联合种植义齿治疗合并牙颌畸形单牙缺失的临床疗效

摘　　要：目的探析针对合并牙颌畸形单牙缺失患者采用正畸联合种植义齿治疗的效果及安全性。方法选取该院2014年6月—2016年6月期间内接收的104例牙颌畸形单牙缺失患者,随机分为观察组与对照组,每组52例,分别接受正畸联合种植义齿治疗、仅种植义齿治疗。对比治疗效果以及治疗前后牙齿功能评分变化情况,观察治疗后并发症发生情况以及牙齿美学满意度。结果观察组治疗总有效率100.00%较对照组88.46%更高(χ2=12.247,P=0.000);观察组牙齿美学总满意率98.08%较对照组84.62%更高(χ2=11.464,P=0.001);观察组总不良反应发生率5.77%较对照组17.31%更低(χ2=6.523,P=0.011);观察组咀嚼功能评分(9.38±0.46)分、语言功能评分(9.24±0.21)分与对照组(7.12±0.39)分、(7.03±0.46)分,差异有统计学意义(t=27.023,31.516,P=0.000 <0.05)。结论针对牙颌畸形单牙缺失患者,采用正畸联合种植义齿治疗后患者牙齿功能及美学情况显著改善,治疗效果确切,且治疗安全性较高,值得推广。Objective To evaluate the efficacy and safety of orthodontic implant denture in patients with single tooth loss with combined dental malformation. Methods A total of 104 patients with single tooth loss who were received during the period from June 2014 to June 2016 were convenient selected and randomly divided into observation group and control group. 52 cases in each group, respectively received orthodontic implant denture treatment and only implant denture treatment. The effects of treatment and changes in dental function scores before and after treatment were compared, and the postoperative complications and the aesthetic satisfaction of the teeth were observed. Results The total effective rate of treatment in the observation group was 100.00% higher than that in the control group 88.46%(χ^2=12.247, P=0.000);the total aesthetic satisfaction rate of the teeth in the observation group was 98.08% higher than that in the control group, 84.62%(χ^2=11.464, P=0.001);the incidence of total adverse reactions was 5.77% in the observation group compared with 17.31% in the control group(χ^2=6.523, P=0.011);the chewing function score (9.38±0.46)points and language function score (9.24±0.21)points in the observation group and the control group (7.12±0.39)points and (7.03±0.46)points, and the difference was statistically significant(t=27.023,31.516,P=0.000 <0.05). Conclusion For patients with single tooth loss of dental malformation, the patient's dental function and aesthetics are significantly improved after orthodontic implant denture treatment. The treatment effect is clear and the treatment safety is high, which is worthy of promotion.
