摘 要:目的了解黏液型铜绿假单胞菌的耐药特征,探究其耐药机制,为临床合理有效的治疗提供依据。方法应用MIC法与K-B分别检测38株黏液型铜绿假单胞菌与282株非黏液型铜绿假单胞菌对常见药物的耐药性,并进行统计学分析。结果黏液型铜绿假单胞菌的耐药性较低,非黏液型铜绿假单胞菌耐药性较高,非黏液型铜绿假单胞菌对复方新诺明的耐药率高达100%,其次是对喹诺酮类耐药率较高,可达61.7%;黏液型铜绿假单胞菌对复方新诺明的耐药率仅51.5%,而对喹诺酮类耐药率也较低,相比仅有31.2%。结论黏液型铜绿假单胞菌的体外耐药率明显低于非黏液型铜绿假单胞菌,但其最主要原因是黏液型铜绿假单胞菌表面的生物膜覆盖,因此临床报告为铜绿假单胞菌时,应为临床批注是否黏液型,以为临床医生更好的选择准确的抗生素提高有力的证据。Objective To understand the drug resistance characteristics of mucoid Pseudomonas aeruginosa and explore its drug resistance mechanism to provide a basis for clinical rational and effective treatment. Methods MIC method and K-B were used to detect the resistance of 38 mucoid Pseudomonas aeruginosa and 282 non-mucoid Pseudomonas aeruginosa to common drugs, and statistical analysis was performed. Results The drug resistance of mucoid Pseudomonas aeruginosa was low, and the resistance of non-mucinous Pseudomonas aeruginosa was higher. The resistance rate of non-mucinous Pseudomonas aeruginosa to compound sulfamethoxazole was as high as 100%. Secondly, the resistance rate to quinolones is up to 61.7%;the resistance rate of mucoid Pseudomonas aeruginosa to compound sulfamethoxazole is only 51.5%, while the resistance rate to quinolones is also lower, compared with only 31.2%. Conclusion In vitro resistance rate of mucoid Pseudomonas aeruginosa was significantly lower than that of non-mucoid Pseudomonas aeruginosa, but the main reason was the biofilm coverage of mucoid Pseudomonas aeruginosa, so the clinical report is Pseudomonas aeruginosa, it should be clinically annotated whether it is a mucus type, which is a strong evidence for clinicians to better choose accurate antibiotics.
