射野分裂技术对宫颈癌术后静态调强放射治疗计划的影响

摘　　要：目的:探讨调强放射治疗中射野分裂技术对宫颈癌大野静态调强计划的影响。方法:选取15例宫颈癌术后患者,每例患者按照分裂射野和非分裂射野分别设计两种调强放射治疗计划,比较两种治疗计划中靶区剂量参数和危及器官剂量参数的差异,以及计划的射野数、机器跳数和执行时间的差异,对比分析两种治疗计划对宫颈癌术后患者静态调强放射治疗计划结果的影响。结果:两种计划的机器跳数、治疗时间、靶区均匀度指数(HI)和直肠受照50 Gy对应体积(V_50)相比,差异有统计学意义(t=-15.385,t=-25.890,t=2.428,t=-4.053;P<0.05);靶区和危及器官的其他受量差异均无统计学意义。结论:非分裂射野技术与分裂射野技术相比较,可以得到相近的剂量分布,同时能有效减少机器跳数,节省治疗时间,提高治疗效率。Objective: To discuss the effect of split-field technique in intensity modulated radiotherapy for static intensity modulated plan of big field of patients with cervical carcinoma. Methods: 15 postoperative patients with cervical carcinoma were selected, two intensity modulated radiotherapy plans were designed for each patient according to splitting field or not. The differences of dosage parameter of target region and dosage parameter of organ at risk, and the differences of the numbers of fields, and the differences of MUs and the execution time between the two plans were compared. And the effects of two treatment plans for the results of static intensity modulated radiotherapy plan of postoperative patients with cervical carcinoma were compared. Results: The differences of MUs, treatment time, homogeneity index(HI) of target region and the corresponding volume which was when the rectum was irradiated by V50 between the two plans were significant(t=-15.385, t=-25.890, t=2.428, t=-4.053, P<0.05). And the differences of other indicators of target region and organ at risk between the two groups were no significant. Conclusion: Compared with the split-field technique, the non-split field technique can not only obtain a similar dose distribution, but also can effectively reduce MUs, and save the time of treatment and improve the efficiency of treatment.
