沙参麦门冬汤联合加倍剂量替加环素对老年多重耐药性呼吸机相关性肺炎患者血清T细胞亚群水平的影响

摘　　要：目的:研究沙参麦门冬汤、加倍剂量替加环素联合治疗老年多重耐药性呼吸机相关性肺炎的临床效果。方法:选取2016年2月至2018年2月医院老年多重耐药性呼吸机相关性肺炎患者81例,按随机数字表法分观察组(n=41)和对照组(n=40)。对照组采用加倍剂量替加环素治疗,观察组于对照组基础上加用沙参麦门冬汤治疗。对比两组疗效、病原菌清除效果、治疗前后血清T细胞亚群(CD3^+、CD4^+)水平。结果:观察组治疗总有效率为90.24%(37/41)、病原菌清除率为87.80%(36/41),较对照组高(P<0.05);治疗后两组免疫功能均不同程度改善,且观察组CD3^+、CD4^+水平较对照组高(P<0.05)。结论:沙参麦门冬汤、加倍剂量替加环素联合治疗老年多重耐药性呼吸机相关性肺炎效果显著,能有效改善患者免疫功能,清除病原菌。Objective:To study the clinical effects of Shashen Maimendong Decoction combined with double-dose tigecycline in the treatment of multidrug-resistant ventilator-associated pneumonia in the elderly.Methods:Eighty-one patients with multi-drug resistant ventilator-associated pneumonia from February 2016 to February 2018 were enrolled.The observation group(n=41)and the control group(n=40)were divided by the random number table method.The control group was treated with double dose of tigecycline,and the observation group was treated with Shashen Maimendong Decoction on the basis of the control group.The efficacy,the pathogen clearance effect,and the serum T cell subsets(CD3^+and CD4^+)levels of the two groups before and after treatment were compared.Results:In the observation group,the total effective rate was 90.24%(37/41)and the pathogen clearance rate was 87.80%(36/41),which was higher than that of the control group(P<0.05).The immune function of the two groups improved after treatment.The levels of CD3^+and CD4^+in the observation group were higher than those in the control group(P<0.05).Conclusion:Shashen Maimeng Decoction combined with double-dose tigecycline is effective in the treatment of multidrug-resistant ventilator-associated pneumonia in the elderly.It can effectively improve the immune function and eliminate pathogens.
