摘 要:目的探讨3D-FLAIR序列磁共振成像(MRI)对单侧梅尼埃病患者前庭积水和耳蜗积水的显示效果。方法对48例符合临床诊断标准的单侧梅尼埃病患者进行鼓室注射钆喷酸葡胺(生理盐水稀释8倍),24小时后使用3.0T磁场强度3D-FLAIR序列进行内耳MRI成像检查,观察前庭积水和耳蜗积水的情况。结果 48例患者中,42例(87.5%)患耳前庭积水阳性,阴性6例(12.5%);耳蜗明确积水5例(10.4%),明确无积水11例(22.9%),无法判定32例(66.7%)。结论 3D-FLAIR序列内耳增强MRI造影检查对前庭积水的显示较好,对耳蜗积水显示欠佳。Objective To study the diagnostic value of 3D-FLAIR MRI sequence in showing endolymphatic hydrops in patients with unilateral Meniere's disease (MD). Methods Forty-eight patients were enrolled. Gd-DTPA was diluted and injected intratympanically in those patients. Then the patients had 3-Tesla MRI imaging at 24 hours after the injection. The endolymphatic hydrops (EH) status was evaluated in the vestibule and the cochlear, respectively. Results In all the 48 patients, 42 ( 87.5%) were detected EH positive in the vestibule, and 6 patients were EH negative. The cochlear EH was positive in only 5 ( 10.4%) patients, and negative in 11 patients, while the cochlear EH status was unclear for 32 patients. Conclusion 3D-FLAIR MRI imaging could detect endolymphatic hydrops effectively in vestibule, but poorly in cochlear in MD patients.
