摘　　要：目的观察火针配合针刺董氏奇穴治疗肩关节周围炎的临床疗效。方法将144例肩关节周围炎患者随机分为观察组和对照组,每组72例。观察组采用火针配合针刺董氏奇穴治疗,对照组采用火针治疗。观察两组不同时间视觉模拟量表(VAS)评分、肩关节功能各项评分变化,并比较临床疗效。结果观察组临床疗效和治愈率明显优于对照组(P<0.05,P<0.01)。两组治疗后和随访VAS评分和肩关节功能各项评分均较同组治疗前明显改善(P<0.01),观察组VAS评分明显低于对照组(P<0.01),肩关节功能各项评分均明显高于对照组(P<0.01)。结论火针配合针刺董氏奇穴治疗肩关节周围炎疗效确切,在镇痛、改善肩关节活动范围、提高日常生活活动能力方面均明显优于单纯火针治疗,显著提高了肩关节周围炎的近期和远期疗效。Objective To observe the clinical efficacy of fire needling plus acupuncture at Dong’s extraordinary points in treating scapulohumeral periarthritis. Method One hundred and forty-four patients with scapulohumeral periarthritis were randomized to observation and control groups, with 72 cases in each group. The observation group received fire needling plus acupuncture at Dong’s extraordinary points and the control group, fire needling alone. The Visual Analogue Scale (VAS) score and the shoulder function item scores were recorded at different times in the two groups. The clinical therapeutic effects were compared between the two groups. Result The clinical therapeutic effect and the cure rate were significantly higher in the observation group than in the control group (P<0.05, P<0.01). The VAS score and the shoulder function item scores significantly changed in both groups at the end of treatment and the follow-up compared with before treatment (P<0.01). After treatment, the VAS score was significantly lower (P<0.01) and the shoulder function item scores were significantly higher (P<0.01) in the observation group than in the control group. Conclusion Fire needling plus acupuncture at Dong’s extraordinary points has a definite therapeutic effect on scapulohumeral periarthritis. It is significantly better than fire needling alone in relieving pain and improving shoulder activity and activities of daily living. It significantly enhances the short-term and long-term therapeutic effect on scapulohumeral periarthritis.
