超声对以餐后腹痛为主要症状的MALS的诊断价值

摘　　要：目的探讨彩色多普勒超声对以餐后腹痛为主要症状的中弓韧带压迫综合征(MALS)的诊断价值。方法对以餐后腹痛为主要症状,疑似腹部血管源性疾病患者临床资料进行回顾性分析,选取经超声筛查疑似MALS病例;另选取医院同期行CT血管造影(CTA)检查腹部正常者100例为对照组。采用彩色多普勒超声诊断仪测量受试者腹腔干动脉起始段管腔内径,腹腔干动脉起始段平静状态、深吸气末、深呼气末的收缩期峰值速度(PSV),计算血流速度变化率,比较两组各超声参数,并分析深呼气末的PSV(PSVe)、血流速度变化率以及两者联合诊断的效能。结果 182例腹部血管源性疾病患者中,经超声检查诊断为MALS阳性89例,诊断灵敏度为93.10%,特异度为91.58%;MALS组管腔内径小于对照组,腹腔干动脉起始段平静状态PSV(PSVc)、深吸气末PSV(PSVi)、PSVe以及血流速度变化率均高于对照组(P <0.05);PSVe和血流速度变化率以及两者联合诊断MALS的受试者工作特征曲线(ROC)下面积(AUC)分别为0.931、0.924和0.952,均有较高诊断效能。结论彩色多普勒超声参数PSVe或血流速度变化率均对MALS具有较高诊断价值。Objective To explore the diagnostic value of color Doppler ultrasonography for middle arch ligament compression syndrome(MALS)with postprandial abdominal pain as the main symptom.MeAods The clinical data of patients with postprandial abdominal pain and suspected abdominal vasculogenic diseases were retrospectively analyzed,and the suspected MALS cases were screened by use of ultrasonography.Another 100 patients with normal abdomen were selected as a control group.Color Doppler ultrasound diagnostic instrument was used to measure the inner diameter of the initial segment of the celiac trunk artery and the peak systolic velocity(PSV)of the initial segment of the celiac trunk artery at the calm state and at the end of both deep inspiration and deep expiration,and the change rate of blood flow velocity was calculated.The ultrasound parameters between the two groups were compared,and the PSV(PSVe),the change rate of blood flow velocity and the combined diagnostic efficiency of the two groups were analyzed.Results Among the 182 patients with abdominal vasculogenic diseases,89 ones were diagnosed as MALS positive by use of ultrasonography,the diagnostic sensitivity was 93.10%,and the specificity was 91.58%;the diameter of the lumen in the MALS group was smaller than that in the control group,while the PSV of the initial segment of the celiac trunk artery at the calm state(PSVc)and that at the end of both deep inspiration and deep expiration(PSVi and PSVe)as well as the change rate of blood flow velocity were higher than those in control group(P<0.05).The area under the receiver operating characteristic curve(ROC)(AUC)in the diagnosis by use of PSVe,change rate of blood flow velocity and both of them was 0.931,0.924 and 0.952,respectively,all indicating high diagnostic efficiency.Conclusion Both the color Doppler ultrasound parameter PSVe and the change rate of blood flow velocity had high diagnostic value for MALS.
