中医活血化瘀联合复荣通脉胶囊口服对糖尿病肢体血管病变患者血流动力学指标的影响

摘　　要：目的:研究中医活血化瘀联合复荣通脉胶囊口服对糖尿病肢体血管病变患者血流动力学指标的影响。方法:研究对象选取我院2017年3月-2018年4月收治的糖尿病合并肢体血管病变患者74例,运用随机数字法将其分为对照组和观察组,每组37例。两组患者均行控制血糖等西医常规治疗的基础上加用复荣通脉胶囊;与此同时观察组联合中医活血化瘀法。疗程3个月。治疗结束后,比较两组患者血糖、血流动力学指标的变化情况以及不良反应发生率。结果:两组患者空腹血糖、糖化血红蛋白均明显小于治疗前,且观察组明显小于对照组,比较差异均有统计学意义(P <0.05);两组患者治疗后MN、ABI及足背温度均明显高于治疗前,且观察组明显高于对照组,比较差异均有统计学意义(P <0.05),治疗期间,观察组不良反应发生率高于对照组,但差异无统计学意义(P> 0.05)。结论:活血化瘀联合荣复通脉胶囊治疗糖尿病肢体血管病变患者能够控制血糖水平的同时明显改善血流动力学指标且不会增加不良反应,对改善患者疾病状况亦具有积极有效的作用,值得在临床上推广。Objective: To study the effect of TCM activating blood circulation and expelling stasis combined with compound Rongrong Tongmai capsules on the hemodynamic parameters of patients with diabetic peripheral vascular disease. Methods: 74 patients with diabetes mellitus complicated with peripheral vascular disease treated in our hospital from March 2017 to April 2018 were selected as the research subjects. They were divided into the control group and the observation group by using random number method, with 37 cases in each group. Both groups of patients were treated with traditional Western medicine such as blood glucose control and were added with Furong Tongmai Capsule;at the same time, the observation group was combined with traditional Chinese medicine for promoting blood circulation and removing blood stasis. Treatment was of 3 months. After the end of treatment, the changes of blood glucose and hemodynamic parameters and the incidence of adverse reactions were compared between the two groups. Results: Fasting blood glucose and glycosylated hemoglobin were significantly lower in the two groups than before treatment, and the observation group was more significant than the control group. The difference was statistically significant (P<0.05). The MN, ABI, and the dorsal temperature of both groups after treatment were both significantly higher than before treatment, and the observation group was significantly higher than the control group, the difference was statistically significant (P<0.05), during the treatment period, the incidence of adverse reactions in the observation group (10.81%) was higher than that of the control group (8.11%), but the difference was not statistically significant (χ2= 0.16, P=0.69). Conclusion: TCM activating blood circulation and removing blood stasis combined with Rongfu Tongmai Capsule could effectively control the level of blood glucose in patients with diabetic peripheral vascular disease while improving hemodynamics without increasing adverse reactions.
