一贯煎加味联合西药治疗复发性阿弗他溃疡合并糖尿病的疗效观察

摘　　要：目的探讨一贯煎加味联合西药治疗复发性阿弗他溃疡(RAU)合并糖尿病的临床疗效。方法选取2017年6月～2018年5月山东省立口腔医院口腔黏膜科诊治的80例糖尿病合并复发性口腔溃疡的患者,随机分为对照组和实验组,各40例。对照组口服维生素C泡腾片,1片/d,复合维生素B,2片/次,3次/d,复方氯己定含漱液含漱,10 mL漱口3 min,3次/d;实验组在此基础上配合服用一贯煎加味治疗,1剂/日,早晚各1次。两组患者均连续连续治疗4周,观察期为12周,观察两组患者的临床疗效。结果治疗1周后,实验组溃疡及充血缩小面积大于对照组,疼痛指数小于对照组,两组比较,差异有统计学意义(P<0.05);治疗12周后,实验组和对照组的总有效率分别为90.0%和70.0%,两组比较,差异有统计学意义;治疗12周后,两组间歇期均延长,复发溃疡数均减少,同治疗前相比,差异有统计学意义(P<0.05),且实验组间歇期长于对照组,复发溃疡数少于对照组,两组比较,差异有统计学意义(P<0.05)。结论一贯煎加味联合西药治疗复发性阿弗他溃疡合并糖尿病具有较好的临床疗效,可改善临床症状和体征,延长溃疡间歇期,减少复发溃疡数,安全性好,具有一定的临床推广应用价值。Objective To investigate the clinical efficacy of Yiguanjian combined with western medicine in the treatment of recurrent aphthous ulcer(RAU)with diabetes.Methods 80 patients of RAU with anxiety and depression in the Shandong Provincial Hospital from April 2017 to May 2018 were randomly divided into test and control groups,and each group had 40 cases.Patients in the control group were administered with vitamin C,1tablet/d,compound vitamin B tablets,2 tablets/time,three times daily and gargling with compound chlorhexidine liquid,10ml/time 3 times/d.Patients in the test group were po administered with Yiguanjian on the basis of the control group,1 dose/time,2 times/d.Patients in two groups were treated for 4 weeks.and followed up for 12 weeks.Results After 1 week’s treatment,the reduced size of ulceration and congestion was larger and the VAS sores were lower than the test group,and there were differences between two groups(P<0.05).After 12 weeks’treatment,the clinical efficacy in the test and control group was 90%and 70.0%,respectively,and there was differences between two groups(P<0.05).After 12 weeks’treatment,the interval in two groups was significantly increased and the ulcer number was significantly decreased in two groups,and the difference was statistically significantly in the same group(P<0.05).After 12 weeks’treatment the interval in the test group was significantly increased and the ulcer number in the test group was significantly decreased than that of the control group,with significant difference between two groups(P<0.05).Conclusion Yiguanjian combined with western medicine is effective in the treatment of RAU with diabetes,can improve clinical symptoms and signs,prolong the interval,reduced the ulcer number with good safety,which has a certain clinical application value.
