消化系统癌症病人情绪状态、幸福感与睡眠质量的相关性研究

摘　　要：[目的]了解消化系统癌症病人情绪状态、睡眠质量与幸福感现状,运用结构方程模型探讨三者之间的相关性。[方法]采用综合性医院焦虑抑郁量表(HADS)、匹兹堡睡眠质量指数(PSQI)与Campbell幸福感指数量表对杭州市某三级甲等医院的220例消化系统癌症病人进行测评。[结果]消化系统癌症病人焦虑、抑郁负性情绪较重,其负性情绪在病人临床分期上有显著差异(F=4.76,2.08;P<0.05),并与睡眠质量呈正相关(r=0.441,0.306;P<0.01),与幸福感指数也呈正相关(r=0.328,0.420;P<0.01);消化系统癌症病人睡眠质量较差,与幸福感指数呈正相关(r=0.621;P<0.01);消化系统癌症病人幸福感指数普遍偏低;焦虑、抑郁负性情绪对睡眠质量、幸福感指数有直接影响,并通过睡眠质量对幸福感指数有间接影响。[结论]消化系统癌症病人焦虑、抑郁负性情绪直接或间接地影响了其睡眠质量与幸福感指数,提示临床护理工作者应重视病人负性情绪的评估,并及时进行心理干预。Objective: To explore the digestive system cancer patients′ emotional state,the sleep quality and the status quo of the happiness,using structural equation model to investigate the correlation among the three. Methods: In the study,220 digestive system cancer patients were investigated with hospital anxiety depression scale(HADS),Pittsburgh sleep quality index(PSQI)and Campbell well-being index scale. Results: Negative emotions like anxiety,depression are bothering the digestive system cancer patients severely,having significant differences in patients with clinical stage(F=4.76,2.08;P <0.05),and positive correlation with sleep quality( r = 0.441,0.306;P <0.01),and also positively correlated with happiness index( r =0.328,0.420, P <0.01).The digestive system cancer patients′ sleep quality was bad,and were positively correlated with happiness index( r =0.621, P <0.621).The digestive system cancer patients happiness index was generally lower.Anxiety and depression of negative mood had a direct effect on sleep quality and happiness index,and through the sleep quality have indirect effects on happiness index. Conclusions: Digestive system cancer patients with anxiety and depression that of negative emotions directly or indirectly affected the sleep quality and happiness index,it prompts clinical nursing workers attaches great importance to the assessment of patients with negative emotions,and psychological intervention in time.
