摘 要:目的分析慢性肾衰竭患者血清Scr、BUN、Cys C、β_2-MG、Hcy的变化及其相关性。方法收集2015年7月至2017年8月我院收治的慢性肾衰竭患者120例为研究组。另选择同期体检的健康人群80例作为对照组。根据CKD分期将患者分为CKD3期组、CKD4期组、CKD5期组。检测各组患者Scr、BUN、Cys C、β_2-MG、Hcy的水平,并采用Pearson相关系数对炎症因子进行相关性分析。结果研究组血清Scr、BUN、CysC、β_2-MG、Hcy水平均明显高于对照组(P<0.05)。研究组患者Scr、BUN、Cys C、β_2-MG及Hcy水平随着病情发展而升高,分期越高,Scr、BUN、Cys C、β_2 MG及Hcy水平越高,差异均具有统计学意义(P <0. 05)。研究组患者血清Scr、BUN与Cys C、β_2-MG及Hcy水平呈正相关(P <0. 05)。Cys C对慢性肾衰竭的灵敏度明显优于β_2-MG和Hcy(P<0.05)。Cys C在慢性CKD 3、4期的灵敏度明显优于β_2-MG和Hcy(P<0.05)。结论慢性肾衰竭患者CysC、Hcy、β_2-MG水平可反映慢性肾衰竭的严重程度,为慢性肾衰竭患者的治疗效果及预后提供参考,临床意义较高。Objective To analyze the changes and correlation among serum Scr,BUN,Cys C,β 2-MG and Hcy in patients with chronic renal failure. Methods We collected 120 patients with chronic renal failure who was treated in our hospital from July 2015 to August 2017 as the study group.Another 80 healthy people in the same period for the physical examination were selected as the control group.According to CKD staging,patients were divided into CKD3 group,CKD4 group,and CKD5 group.The levels of Scr,BUN,Cys C,β 2-MG and Hcy were then detected in each group.The correlations among inflammatory factors were analyzed using Pearson′s correlation coefficient. Results The serum levels of Scr,BUN,Cys C,β 2-MG,and Hcy in the study group were significantly higher than those in the control group( P <0.05).The levels of Scr,BUN,Cys C,β 2-MG,and Hcy in the study group increased along with the disease progressed.he levels of Scr,BUN,Cys C,β 2-MG,and Hcy were getting higher with the staging of disease progressed( P < 0.05 ).The serum Scr,BUN were positively correlated with Cys C,β 2-MG and Hcy levels in the study group( P <0.05).The sensitivity of Cys C to chronic renal failure was significantly better than that of β 2-MG and Hcy( P <0.05).The sensitivity of Cys C in chronic CKD stage 3 and 4 was significantly better than that of β 2-MG and Hcy( P <0.05). Conclusion The levels of Cys C,Hcy,and β 2-MG in patients with chronic renal failure can reflect the severity of disease,providing a reference for the treatment effect and prognosis of patients,which have significant clinical significance.
