成都基层医疗机构老年卫生服务现状及影响因素调查

摘　　要：目的:了解成都基层医疗机构老年卫生服务现状并分析其影响因素,为加快老年康复护理事业建设提供参考。方法:2016年对成都市21个辖区(市、县)的注册医疗机构进行问卷调查。结果:共调查568家基层医疗机构,老年门诊及住院人次二级以下机构远高于二、三级机构;老年科患者住院费及住院时间高于均同期全院平均水平;8.98%的受访机构开设了老年专科门诊,老年病房开设率三级机构高于二级及以下机构;二级机构老年科医生日均住院服务强度分别是三级机构的1.8倍、二级以下机构的2.8倍,二级以下机构护士日均住院服务强度分别是三级机构的1.8倍、二级机构的3.3倍;基层机构开展老年医学继续教育和科研不足,受访机构中均无老年综合评估员和软件;Logistic回归分析显示老年综合征的评估是机构开展老年住院医疗服务的独立影响因素。结论:成都基层医疗机构老年服务存在供给不平衡、资源分配不合理、技能型人力资源匮乏、适宜技术开展不充分现象,尚需优化医疗服务资源配置,开展老年综合评估工作,促进老年医疗服务进一步改善。Objective:To understand the current situation of geriatric health service in grass-roots medical institutions in Chengdu and analyze its influencing factors,so as to provide reference for speeding up the construction of geriatric rehabilitation nursing.Methods:A questionnaire survey was conducted among 21 registered medical institutions in Chengdu in 2016.Results:A total of 568 grass-roots medical institutions were surveyed.The proportion of outpatient and hospitalized elderly patients in institutions below secondary level was much higher than that in secondary or tertiary institutions;the hospitalization fee and hospitalization time for elderly patients were higher than the average level of the whole hospital in the same period;8.98%of the respondents set up special outpatient clinics for the elderly,12.50%of the respondents set up geriatric wards,and the opening rate of tertiary institutions was higher than that in secondary or below secondary level institutions.The average daily hospitalization service intensity of geriatricians in secondary institutions was 1.8 times of that in tertiary institutions and 2.8 times of that in institutions below secondary level.The average daily hospitalization service intensity of nurses in institutions below secondary level was 1.8 times of that in tertiary institutions and 3.3 times of that in secondary institutions respectively.Conclusion:There are some problems in the elderly service in Chengdu grass-roots medical institutions,such as unbalanced supply,unreasonable resource allocation,lack of skilled human resources and inadequate development of appropriate technology.It is necessary to optimize the allocation of medical service resources,carry out comprehensive assessment of the elderly,and promote the further improvement of the elderly medical service.
