理气宽胸活血方联合法舒地尔治疗心脏X综合征疗效及对患者心肌微循环、血管内皮细胞功能的影响

摘　　要：目的:观察理气宽胸活血方联合法舒地尔治疗心脏X综合征的疗效及对心肌微循环、血管内皮细胞功能的影响。方法:将120例心脏X综合征患者随机分为两组,对照组60例予法舒地尔治疗,治疗组60例在对照组治疗基础上加自拟理气宽胸活血方治疗,疗程2周。比较两组治疗前及治疗后2周后中医症状(胸闷、胸痛、心悸、气短)积分变化和心绞痛症状治疗有效率,静息心肌灌注断层扫描阳性率,血清一氧化氮、内皮素水平的变化。结果:两组用药2周后胸闷、胸痛、心悸、气短症状积分及总分均较本组用药前明显降低(P<0.05),且治疗组低于对照组(P<0.05),心绞痛治疗有效率相比,治疗组(86.7%)明显高于对照组(55.0%),差异有统计学意义(P<0.05),静息心肌灌注断层扫描(ECT)阳性率、内皮素(ET-1)水平较治疗前明显下降,一氧化氮(NO)水平明显升高(P<0.05),且治疗组与对照组相比,差异均有统计学意义(P<0.05)。治疗过程中两组均未发生不良反应。结论:理气宽胸活血方联合法舒地尔能够通过改善血管内皮细胞舒缩功能来改善心肌微循环,减少心绞痛症状的反复发作,提高患者生活质量,临床疗效显著。Objective:To observe the curative effect of s Liqi Kuanxiong Huoxue recipe combined with fasudil in the treatment of cardiac syndrome X and its effect on myocardial microcirculation and vascular endothelial cell function. Methods:120 patients with cardiac syndrome X were randomly divided into 2 groups. 60 patients in the control group were treated with fasudil. 60 patients in the treatment group were treated with the prescription of Liqi Kuanxiong Huoxue recipe on the basis of the control group for 2 weeks. The scores of TCM symptoms (chest tightness,chest pain,palpitations,shortness of breath) and angina pectoris were compared before and 2 weeks after treatment,the positive rate of resting myocardial perfusion scan,serum levels of nitric oxide and endothelin. Results:After 2 weeks of treatment,the scores of chest tightness,chest pain,palpitations,shortness of breath and total scores were significantly lower than those in this group ( P <0.05),and the treatment group was lower than the control group ( P <0.05),and the two groups were given medication. The effective rate of angina pectoris treatment was significantly higher in the treatment group (86.7%) than in the control group (55.0%). The difference was statistically significant ( P <0.05),indicating that the clinical efficacy of the treatment group was better than that of the control group.The positive rate of myocardial perfusion tomography (ECT) and endothelin (ET-1) levels were significantly lower than those before treatment,and the level of nitric oxide (NO) was significantly increased ( P <0.05). Compared with the control group,the difference was statistically significant ( P <0.05). No adverse reactions occurred in the two groups during the treatment. Conclusion:Liqi Kuanxiong Huoxue recipe combined with fasudil can improve myocardial microcirculation,reduce recurrent episodes of angina pectoris,improve the quality of life of patients with vasomotor dysfunction,improve clinical quality,and have high clinical efficacy and safety.
