小儿泌尿系感染的临床特点及病原菌耐药性分析

摘　　要：目的了解厦门市妇幼保健院2013—2017年儿童泌尿系感染病原菌分布及耐药性情况。方法回顾分析我院2013年1月—2017年12月儿童尿液培养及药敏试验结果。结果儿童泌尿系感染主要以革兰阴性杆菌感染为主,病原菌构成比前五位分别为大肠埃希菌463例(54.03%)、肺炎克雷伯菌92例(10.74%)、粪肠球菌70例(8.17%)、屎肠球菌49例(5.72%)、奇异变形杆菌21例(2.45%)。大肠埃希菌对碳青霉烯类敏感率98.5%,产ESBLs菌株检出率高达42.98%,肠球菌尚未发现对万古霉素耐药的菌株。结论儿童泌尿系感染的主要致病菌为大肠埃希菌,产ESBLs菌株检出率高,且肠球菌感染有所上升,临床医师应引起重视,合理使用抗菌药物。Objective To understand the distribution of pathogens and drug resistance of children with urinary tract infection in Xiamen Maternity and Child Health Care Hospital from 2013 to 2017. Methods The results of children's urine culture and drug susceptibility testing in our hospital from January 2013 to December 2017 were retrospectively analyzed. Results The infection of urinary tract infection in children was mainly Gramnegative bacillus infection. The pathogenic bacteria consisted of E. coli 463 (54.03%), Klebsiella pneumoniae 92 (10.74%) and Enterococcus faecalis 70 (8.17%). Enterococcus faecium 49 (5.72%), Proteus mirabilis 21 (2.45%). Drug sensitivity test presented :the sensitivity rate of E. coli to carbapenems was 98.5%, the detection rate of ESBLs-producing strains was as high as 42.98%. Enterococcus had not been found to be resistant to vancomycin. Conclusion The main pathogen of Children's urinary tract infection is Escherichia coli, the detection rate of ESBLs-producing strains is high, and enterococci infection has risen. Clinicians should pay attention to rational use of atibacterial drugs.
