耳内镜下鼓膜切开置管联合鼻内镜下腺样体切除治疗儿童分泌性中耳炎的临床治疗效果分析

摘　　要：目的研究探讨耳内镜下鼓膜切开置管联合鼻内镜下腺样体切除治疗儿童分泌性中耳炎的临床治疗效果。方法从2017年8月—2018年8月该院收治的儿童分泌性中耳炎患儿当中,根据盲选的方法从中选择60例进行研究,根据治疗的方法和平均分组的方式,将其分为对照组和观察组。对照组采用耳内镜下鼓膜切开置管的方法进行治疗,而观察组则采用耳内镜下鼓膜切开置管联合鼻内镜下腺样体切除的方法进行治疗,对比两组研究对象治疗的效果。结果通过对比发现,对照组中治疗效果为显效的例数为14例,治疗效果为有效的例数为11例,治疗的总体有效率为83.3%;观察组中治疗效果为显效的例数为16例,治疗效果为有效的例数为12例,治疗的总体有效率为93.3%。观察组研究对象治疗的效果要优于对照组研究对象治疗的效果(t=2.43,P<0.05);观察组研究对象积液持续存在的时间为(7.49±1.23)d,耳鸣耳闷消失的时间为(9.42±1.33)d,鼓膜愈合的时间为(11.27±1.57)d,听力恢复正常的时间为(23.87±1.34)d;对照组研究对象积液持续存在的时间为(15.81±2.36)d,耳鸣耳闷消失的时间为(17.36±2.31)d,鼓膜愈合的时间为(21.57±1.96)d,听力恢复正常的时间为(37.46±2.56)d,两组之间的数据对比,差异有统计学意义(P<0.05)。结论耳内镜下鼓膜切开置管联合鼻内镜下腺样体切除治疗儿童分泌性中耳炎具有良好的治疗效果,值得在临床上进行推广。Objective To investigate the clinical effect of endoscopic tympanotomy combined with endoscopic adenoidectomy for children with secretory otitis media. Methods From August 2017 to August 2018, among children with secretory otitis media admitted to our hospital, 60 cases were selected according to blind selection method. According to the treatment method and the average grouping method, they were divided into two groups: the control group and observation group. The control group was treated with endoscopic tympanic membrane incision, while the observation group was treated with endoscopic tympanotomy and endoscopic adenoidectomy. The two groups were compared of the effect of treatment. Results By comparison, the number of cases in which the therapeutic effect was markedly effective was 14 cases, the number of cases with effective treatment was 11 cases, and the overall effective rate of treatment was83.3%. The number of cases in the observation group was markedly effective in 16 cases, the number of effective treatments was in 12 cases, and the overall effective rate of treatment was 93.3%. The effect of treatment in the observation group was better than that in the control group(t=2.43,P<0.05);the duration of effusion in the observation group was(7.49 ±1.23)d, and the tinnitus disappearance time was(9.42 ±1.33)d, the time of tympanic membrane healing was(11.27±1.57)d, the time of normal hearing recovery was(23.87±1.34)d, and the duration of effusion persistence in the control group was(15.81±2.36)d, the time of tinnitus and ear suffocation disappeared(17.36±2.31)d, the time of tympanic membrane healing was(21.57±1.96)d, and the time of normal hearing recovery was(37.46±2.56)d. The data comparison between the two groups had statistical significance(t=2.43,P<0.05). Conclusion Endoscopic tympanic incision combined with endoscopic adenoidectomy for the treatment of children with secretory otitis media has a good therapeutic effect and is worthy of clinical promotion.
