乙肝肝硬化合并糖尿病临床特点及预后分析

摘　　要：目的探讨合并糖尿病对乙肝肝硬化患者病情进展及预后的影响。方法选取南昌市第九医院收治的184例乙肝肝硬化患者为研究对象,按照是否合并糖尿病分为两组,糖尿病组(48例)与非糖尿病组(136例),对比两组患者入院时血清谷丙转氨酶、总胆红素、白蛋白及凝血酶原时间水平及出院后随访期间两组患者并发症出现及预后情况。结果乙肝肝硬化合并糖尿病组血清谷丙转氨酶、总胆红素及凝血酶原时间明显高于非糖尿病组,差异有统计学意义(P<0.05),乙肝肝硬化合并糖尿病患者血清白蛋白水平低于非糖尿病组,差异有统计学意义(P<0.05)。对两组患者随访发现,乙肝肝硬化合并糖尿病组出现上消化道出血、肝性脑病、原发性腹膜炎、原发性肝癌概率高于非糖尿病组,差异有统计学意义(P<0.05)。糖尿病组死亡率明显高于非糖尿病组,差异有统计学意义(P<0.05)。结论乙肝肝硬化合并糖尿病患者,肝功能损坏严重,远期肝硬化并发症发生率高,预后不佳。Objective To study the effect of diabetes mellitus on the progression and prognosis of HBV-related cirrhosis. Methods 184 patients with HBV-related cirrhosis diagnosed in Ninth Hospital of NanChang were selected, combined with diabetes mellitus (DM) group (48 cases) and nondiabetic group (136 cases). Serum glutamase, total bilirubin, albumin and prothrombin time were compared between the two groups, and complications and prognosis of patients were observed after discharge. Results Serum glutamase, total bilirubin and prothrombin time in the DM group were significantly higher than those in the non-diabetic group (P<0.05). Serum albumin levels in patients combined with diabetes were significantly lower than those in the non-diabetic group (P<0.05). In follow-up study, the incidence of upper gastrointestinal bleeding, hepatic encephalopathy, primary peritonitis and primary liver cancer in the DM group was significantly higher than those in the non-diabetic group (P<0.05). Conclusion Patients with HBV-related cirrhosis combined with diabetes mellitus have serious liver function damage, high incidence of long-term cirrhosis complications and poor prognosis.
