腹腔镜下肾囊肿去顶减压翻转术和去顶减压术的比较

摘　　要：目的比较腹腔镜下肾囊肿去顶减压翻转术和去顶减压术两种术式治疗肾囊肿的临床价值。方法回顾性分析本科2010年1月至2014年3月55例腹腔镜下肾囊肿去顶减压术(A组)和62例腹腔镜下肾囊肿去顶减压翻转术(B组)的临床资料并进行统计比较,统计分析两种术式术前,术中和术后的数据资料。结果所有手术均成功。A组的手术时间、术中出血量、引流管拔除时间、术后住院天数、复发率分别是(73.46±3.42)min、(9.25±1.67)ml、(52.27±5.48)h、(5.03±0.07)d、7.27%(4/55),B组分别是(85.64±2.33)min、(9.57±1.38)ml、(53.46±4.78)h、(5.05±0.06)d、0(0/62),A组的手术时间少于B组的手术时间,复发率大于B组的复发率,差异有统计学意义(P<0.05),两组术中出血量、引流管拔除时间、术后住院天数相比差异无统计学意义。结论腹腔镜肾囊肿去顶减压翻转术复发率明显优于腹腔镜下肾囊肿去顶减压术,是治疗肾囊肿安全、更有效的新术式。Objective To evaluate efficacy of laparoscopic unroofing-eversion of renal cysts and its comparison of laparoscopic unroofing of renal cysts. Methods A retrospective analysis of database from January 2010 to March 2014. The database comprises information for demographic, clinical, imaging, preoperative, intraoperative, and postoperative data. A comparison of two groups ( group A, 55 cases, laparoscopic unroofing of renal cysts;group B, 62 cases, laparoscopic unroofing-eversion of renal cysts) were done for operating time, estimated blood loss, the mean time of extubation, postoperative hospital stay and recurrence rate. Results All operations were successfully performed. The operating time, estimated blood loss, the mean time of extubation, postoperative hospital stay and recurrence rate of group A were (73.46±3.42) min,(9.25±1.67)ml,(52.27±5.48) h,(5.03±0.07)d, 7.27%(4/55), the data of group B were (85.64±2.33) min,(9.57±1.38)ml,(53.46±4.78) h,(5.05± 0.06) d, 0(0/62). There were statistical differences of recurrence rate between the two groups (P<0.05), operating time, estimated blood loss and the mean time of extubation, postoperative hospital stay were no statistical differences between the two groups. Conclusion Laparoscopic unroofing-eversion of renal cysts had lower recurrence rate than laparoscopic unroofing of renal cysts, it is a safe and more efficacy treatment of renal cysts.
