产妇胎膜早破后新生儿感染的护理干预效果分析

摘　　要：目的分析产妇胎膜早破发生新生儿感染的有效综合护理干预效果。方法收集产科2017年1月至2018年1月住院分娩的胎膜早破产妇400例,根据随机数字表法随机分为观察组和对照组,每组200例。对照组产妇给予常规护理,观察组在常规护理基础上给予有效综合护理干预。比较2组产妇生产方式、宫内感染情况、产褥期感染率以及新生儿结局。结果观察组自然分娩率为68.50%高于对照组的54.00%,差异有统计学意义(P<0.05);观察组产妇总感染率为26.00%低于对照组的39.50%,差异有统计学意义(P<0.05);观察组新生儿Apgar评分为(9.08±0.44)分高于对照组的(8.26±0.68)分,体重为(3.45±0.48)kg大于对照组的(2.43±0.34)kg,新生儿感染率、胎儿窘迫率和新生儿窒息率分别为22.50%、11.00%和2.50%低于对照组的32.73%、20.00%和8.00%,差异均有统计学意义(P<0.05)。结论胎膜早破产妇进行有效综合护理干预,可增加产妇自然分娩率,降低产妇及新生儿感染率,改善母婴结局。Objective To investigate the effects of comprehensive nursing intervention on neonatal infection in puerpera with premature rupture of membranes.Methods A total of 400 puerpera with premature rupture of membranes who were admitted and treated in our hospital from January 2017 to January 2018 were enrolled in the study,who were divided into observation group(n=200)and control group(n=200)according to random number table method.The puerpera in control group received routine nursing,however,the puerpera in observation group,on the basis of control group,received effective comprehensive nursing intervention.The maternal delivery modes,intrauterine infection rate,puerperal infection rate and neonatal outcomes were observed and compared between the two groups.Results The natural delivery rate in observation group was 68.50%,which was significantly higher than that(54.00%)in control group(P<0.05).The overall infection rate in observation group was significantly lower than that in control group(26.00%vs 39.50%,P<0.05).The Apgar scores of neonates in observation group were higher than those in control group[(9.08±0.44)points vs(8.26±0.68)points],and the body weight of neonates in observation group was higher than that in control group[(3.45±0.48)kg vs(2.43±0.34)kg],and the neonatal infection rate,fetal distress rate and neonatal asphyxia rate in observation group were 22.50%,11.00%,2.50%,respectively,which were significantly lower than those(32.73%,20.00%,8.00%,respectively)in control group(P<0.05).Conclusion The effective comprehensive nursing intervention for puerpera with premature rupture of membranes can increase the natural delivery rate,reduce the maternal and neonatal infection rates,improve the maternal-infant outcomes.
