胸腰椎骨折患者采用改良微创经皮椎弓根螺钉的临床研究

摘　　要：目的探讨改良微创经皮椎弓根螺钉治疗胸腰椎骨折的临床效果。方法选取2017年2月至2018年2月河南宏力医院收治的86例胸腰椎骨折患者作为研究对象,随机数表法分为观察组和对照组。其中对照组43例,应用开放椎弓钉治疗;观察组43例,应用改良微创经皮椎弓根螺钉治疗,比较两组患者的临床指标以及治疗效果。结果观察组术口长度、手术时间、住院时间、下床活动时间和术中出血量各项指标优于对照组,差异有统计学意义(P <0.05);两组患者的椎间隙高度、椎体前缘高度与矢状位指数均比术前有明显增加,后凸Cobb角与VAS评分比术前明显减少(P <0.05),同时观察组术后疼痛评分低于对照组(P <0.05)。结论在胸腰椎骨折患者的临床治疗中,采用改良微创经皮椎弓根螺钉治疗,固定效果理想,术后疼痛轻,有助于促进患者恢复,值得应用。Objective To investigate the clinical effect of modified minimally invasive percutaneous pedicle screws in the treatment of thoracolumbar fractures. Methods 86 patients with thoracolumbar fractures admitted to Henan Hongli Hospital from February 2017 to February 2018 were selected as the research objects and randomly divided into observation group and control group. 43 cases in the control group were treated with open pedicle screws. 43 cases in the observation group were treated with modified minimally invasive percutaneous pedicle screws, and the clinical indexes and therapeutic effects of the two groups were compared. Results The observation group was superior to the control group in terms of the length of operation mouth, operation time, hospitalization time, ambulation time and intraoperative blood loss, and the differences were statistically significant (P < 0.05). The intervertebral space height, anterior vertebral body height and sagittal index of the two groups of patients were significantly higher than those before operation, and the Cobb angle and VAS score of kyphosis were significantly lower than those before operation (P < 0.05), while the postoperative pain score of the observation group was lower than that of the control group (P < 0.05). Conclusion In the clinical treatment of thoracolumbar fracture patients, the modified minimally invasive percutaneous pedicle screw therapy has ideal fixation effect, less postoperative pain, which is helpful to promote the recovery of patients, and is worthy of application.
