摘 要:目的探究急性发作期哮喘患儿血清IL-4、IFN-γ、TGF-β1水平变化与肺功能的关系。方法回顾性分析2016年4月至2018年6月在我院就诊的急性发作期哮喘患儿82例临床资料并设为观察组,选取同期于我院健康体检无哮喘儿童60例设置为对照组。统计比较两组儿童血清IL-4、IFN-γ、TGF-β1表达水平、肺功能指标,采用Pearson法分析相关性。结果观察组儿童血清IL-4水平显著高于对照组,IFN-γ、TGF-β1均显著低于对照组,差异具有统计学意义(P <0.05);观察组儿童肺功能指标FVC、FEV_1、FEV_1/FVC、PEF、FEF_(25)、FEF_(50)、FEF_(75)显著低于对照组(P <0. 05);重度患儿血清IL-4明显高于中、轻度患儿(q=9.950,q=19.990),中度患儿血清IL-4明显高于轻度患儿(q=10.040),三种不同程度患儿血清IL-4水平差异具有统计学意义(F=33.678);中、重度患儿血清IFN-γ明显低于轻度患儿(q=3.463,q=5. 534),重度患儿血清TGF-β1明显低于轻度患儿(q=4.455,P <0.05);重度患儿血清IFN-γ、TGF-β1与中度患儿差异无统计学意义(q=2.602,q=2.013),中度患儿血清TGF-β1与轻度患儿差异无统计学意义(q=1.712,P>0. 05);观察组儿童血清IL-4与肺功能FVC、FEV_1、FEV_1/FVC、PEF、FEF_(25)、FEF_(50)、FEF_(75)成负相关,血清IFN-γ与肺功能FVC、FEV_1、FEV_1/FVC、PEF、FEF_(25)、FEF_(50)、FEF_(75)呈正相关,血清TGF-β1与肺功能FVC、FEV_1、FEV_1/FVC、PEF、FEF_(25)、FEF_(50)、FEF_(75)呈正相关(P<0. 05)。结论血清IL-4、IFN-γ、TGF-β1水平与支气管哮喘发病密切相关,与患者肺功能存在相关性,可对临床哮喘诊断治疗提供指导。Objective To explore the relationship between changes of serum levels of IL-4,IFN-γand TGF-β1 and the lung function in children patients with acute attack of asthma.Methods The clinical data of 82 children patients with acute attack of asthma in our hospital from April,2016 to June,2018 were retrospectively analyzed and as the observation group.Another 60 cases of children without asthma in our hospital for physical examination during the same period were selected as the control group.The expression levels of serum IL-4,IFN-γand TGF-β1 and lung function indexes were compared between the two groups,and the correlation was analyzed by Pearson method.Results The level of serum IL-4 in the observation group was significantly higher than those in the control group,while the levels of serum IFN-γand TGF-β1 in the observation group were significantly lower than those in the control group(P<0.05).The lung function indexes of FVC,FEV1,FEV1/FVC,PEF,FEF25,FEF50 and FEF75 in the observation group were significantly lower than those in the control group(P<0.05).Serum IL-4 level in severe children patients was significantly higher than that in moderate and mild children patients(q=9.950,q=19.990),and serum IL-4 level in moderate children patients was significantly higher than that in mild children patients(q=10.040),and there was a significant difference in serum IL-4 level among children patients for three degrees(F=33.678).Serum IFN-γlevel in moderate and severe children patients was significantly lower than that in mild children patients(q=3.463,q=5.534),and serum TGF-β1 level in severe children patients was significantly lower than that in mild children patients(q=4.455)(P<0.05).There was no difference in the serum IFN-γand TGF-β1 levels between severe children patients and moderate children patients(q=2.602,q=2.013),and there was no significant difference in the serum TGF-β1 level between moderate children patients and mild children patients(q=1.712)(P>0.05).Serum IL-4 in the observation group was negatively co
