摘  要:目的探讨血清簇集蛋白(Clusterin)、水通道蛋白4(AQP4)、载脂蛋白H(ApoH)与2型糖尿病(T2DM)患者认知功能障碍的相关性。方法选取2016年1月至2018年1月期间的208例T2DM患者作为研究对象。根据蒙特利尔认知评估量表(MoCA)评分将T2DM患者分为认知功能障碍组(≤26分,n=121)和认知功能正常组(≥26分,n=87)。选取同期体检的健康人100例作为对照组。比较各组间基本资料、血清Clusterin、AQP4、ApoH水平,并进行Pearson相关分析与Logistic多元回归相关分析。结果三组间的性别比例、年龄、受教育年限、BMI、HDL-C等差异无统计学意义(P>0.05),认知功能障碍组糖尿病病程、FPG、HbAlc、TC、TG、LDL-C、ApoH、MoCA均显著高于认知功能正常组和对照组,Clusterin、AQP4显著低于认知功能正常组和对照组(P <0. 05)。认知功能正常组糖尿病病程、FPG、HbAlc、TC、TG、LDL-C、ApoH、MoCA均显著高于对照组,Clusterin、AQP4显著低于对照组(P<0.05)。随着T2DM患者认知功能障碍程度的加重,血清Clusterin、AQP4水平不断降低,ApoH不断提高(P <0.05)。经过Pearson相关性分析,Clusterin、AQP4与T2DM患者MoCA评分均呈正相关,ApoH与其呈负相关(P<0.05),ApoH与T2DM患者Clusterin、AQP4均呈负相关(P<0.05)。经过Logistic多元逐步回归分析,Clusterin、AQP4是T2DM患者发生认知功能障碍的保护因素,ApoH是其独立危险因素。结论 Clusterin、AQP4、ApoH均与T2DM患者认知功能障碍有密切关系,对T2DM患者认知功能障碍的早期发现和预防有重要临床意义。Objective To investigate the correlation between serum cluster protein,aquaporin-4,apolipoprotein H and cognitive impairment in type 2 diabetic mellitus(T2DM)patients.Methods 208 patients with T2DM treated in our hospital from January,2016 to January,2018 were selected for the study.According to Montreal Cognitive Assessment scale(MoCA),T2DM patients were divided into two groups:the cognitive dysfunction group(≤26 points)and the normal cognitive function group(≥26 points).100 healthy people were also selected as the control group.The basic data and serum Clusterin,AQP4,ApoH levels were compared among groups.Pearson correlation analysis and logistic regression analysis were performed.Results There were no significant difference in the sex ratio,age,education years and HDL-C among the three groups(P>0.05).Compared with the normal cognitive function group and control group,the course of diabetes,FPG,HbAlc,TC,TG,LDL-C,ApoH,and MoCA of the cognitive dysfunction group were significantly higher,while Clusterin and AQP4 were significantly lower(P<0.05).Compared with the control group,the course of diabetes,FPG,HbAlc,TC,TG,LDL-C,Clusterin,AQP4,ApoH,and MoCA in the normal cognitive function group were significantly higher,while Clusterin and AQP4 were significantly lower(P<0.05).With the increasing severity of cognitive dysfunction in T2DM patients,the level of serum Clusterin and AQP4 decreased,while ApoH increased(P<0.05).By Pearson correlation analysis,there was a positive correlation between cluster protein,aquaporin-4 and MoCA score,a negative correlation between ApoH and MoCA score in T2DM patients(P<0.05),also,there was a positive correlation between ApoH and Clusterin,AQP4 in T2DM patients(P<0.05).There was a negative correlation between ApoH and Clusterin,AQP4 in T2DM patients(P<0.05).Logistic multivariate stepwise regression analysis showed that Clusterin and AQP4 were protective factors for cognitive dysfunction in T2DM patients,and ApoH was an independent risk factor.Conclusion There is a close relationship
