摘  要:目的探讨Mini-CEX在皮肤科实习中临床实践教学的应用价值。方法选取2017年9月—2018年11月在我院皮肤科实习的26名美容及临床专业实习生作为研究对象。由4名专业培训过的教师(高年资住院医师或主治医师)进行实习带教,实习周期为2周。利用Mini-CEX评估系统进行4次测评,比较入科、出科时的教学效果。结果共完成评估155人次,平均每人接受为(5.96±0.56)次测评,平均用时(20.36±3.45)min,反馈平均用时(8.26±1.58)min。入科与出科时实习生各项目的得分为[问诊能力:入科(3.00±1.36)分vs.出科(7.35±0.96)分,t=13.069;查体能力:入科(3.65±1.07)分vs.出科(5.35±1.41)分,t=9.297;临床诊断:入科(3.96±1.16)分vs.出科(5.46±1.55)分,t=6.009;治疗方案:入科(4.46±0.97)分vs.出科(6.58±1.04)分,t=6.606;操作能力:入科(3.31±1.46)分vs.出科(7.12±0.93)分,t=10.346;沟通能力:入科(4.15±0.86)分vs.出科(5.62±1.27)分,t=5.718;整体评价:入科(4.11±1.01)分vs.出科(6.50±1.25)分,t=11.071],出科时较入科时各项评估指标均显著提高,差异具有统计学意义(P <0.01)。结论 Mini-CEX评估体系在皮肤科临床实习教学中取得较好效果。Objective To explore the application value of Mini-CEX in clinical practice teaching in dermatology practice. Methods 26 interns of cosmetology and clinical specialty from September 2017 to November 2018 in dermatology department of our hospital were selected as the subjects of study. 4 professionally trained teachers (senior resident or attending physician) were trained for two weeks. Mini-CEX evaluation system was used to conduct four evaluations to compare the teaching effects of admission and departure. Results A total of 155 evaluations were completed, with an average of (5.96±0.56) times of evaluations per person. The average time spent was (20.36±3.45) minutes, and the average time spent for feedback was (8.26±1.58) minutes. The scores of each item of interns at admission and discharge were [interrogation ability: admission (3.00±1.36) points vs. discharge (7.35±0.96) points, t=13.069;physical examination ability: admission (3.65±1.07) points vs. discharge (5.35±1.41) points, t=9.297;clinical diagnosis: admission (3.96±1.16) points vs. discharge (5.46±1.55)points, t=6.009;treatment plan: admission (4.46±0.97) points vs. discharge (5.46±1.55) points, t=6.009;operational ability: admission (3.31±1.46) points vs. discharge (7.12±0.93) points, t=10.346;communication skills: admission (4.15±0.86) points vs. discharge (5.62±1.27) points, t=5.718;overall evaluation: admission (4.11±1.01) points vs. discharge (6.50±1.25) points , t=11.071] The evaluation indexes at the time of discharge were significantly higher than those at the time of admission, and the difference was statistically significant (P < 0.01). Conclusion The Mini-CEX evaluation system in dermatology achieved better effect in clinical practice teaching.
