自体血液回输技术应用于非体外循环冠状动脉旁路移植术的效果

摘　　要：目的探讨非体外循环冠状动脉旁路移植术(OPCAB)中应用自体血液回输技术的临床效果。方法对2017年2月～2018年2月在我院接受OPCAB治疗的85例患者的临床资料进行回顾分析。按照是否采用自体血液回输技术分为观察组(自体血回输)和对照组(未应用),观察比较两组术后24 h血常规指标水平,并测定术后引流量、异体血输注量及输血不良反应情况。结果两组患者的血红蛋白、红细胞比容、红细胞计数、血小板计数及术后引流量等指标比较,差异均无统计学意义(P>0.05);观察组的异体血输注量、输血不良反应发生率显著低于对照组,差异均有统计学意义(P<0.05)。结论将自体血液回输技术应用于OPCAB治疗中,可有效减少异体血输注量和输血不良反应发生,对患者血常规无明显影响,且安全性较高,值得临床应用。Objective To investigate the clinical effect of autologous blood transfusion in off-pump coronary artery bypass grafting (OPCAB).Methods The clinical data of 85 patients receiving OPCAB from February 2017 to February 2018 in our hospital were analyzed retrospectively.Patients were divided into observation group (application) and control group (non-application) according to whether or not autologous blood transfusion technique was used,45 cases and 40 cases respectively.The Blood Routine Indexes of the two groups were compared 24 hours after operation,and the postoperative drainage volume and allogeneic blood transfusion volume were measured to master the adverse reactions of blood transfusion in the two groups.Results There was no significant difference in hemoglobin (Hb),hematocrit (Hct),red blood cell count (RBC),platelet count (PLA) and postoperative drainage between the two groups ( P > 0.05for all).However,the amount of allogeneic blood transfusion and the incidence of adverse reactions in the observation group were significantly lower than those in the control group ( P < 0.05for both).Conclusion The application of autologous blood transfusion technology in OPCAB treatment can effectively reduce the amount of allogeneic blood transfusion and the occurrence of adverse reactions of blood transfusion.It has no obvious effect on the blood routine of patients and is safe and worthy of clinical application.
