双重建钢板与解剖锁定钢板治疗肱骨中下段螺旋型骨折疗效比较

摘  要:目的评价采用双重建钢板与解剖锁定钢板治疗肱骨中下段螺旋型骨折的临床效果。方法应用肱骨远端后外侧锁定钢板和双重建钢板内固定治疗肱骨中下段螺旋型骨折44例,AO分型B1型28例,A1型16例,其中男31例,女13例,年龄21~65岁。其中解剖锁定钢板组19例,双重建钢板组25例。比较两组手术时间、出血量、骨折愈合时间,并发症以及术后功能恢复等情况。结果44例患者均获得随访,平均随诊11.3个月,骨折愈合率100%。解剖锁定钢板组手术时间(74.53mi±9.92)min,双重建钢板组(92.48±8.93)min(P<0.05);解剖锁定板组术中出血量(106.84±22.12)m,双重建钢板组(139.20±32.26)ml。两组骨折愈合时间、术后6个月肘关节活动度差异无统计学意义(P>0.05)。结论解剖锁定钢板内固定治疗肱骨中下段螺旋型骨折手术操作简单,出血少,手术时间短,是治疗肱骨中下段螺旋型骨折的有效方法。Objective To investigate the curative effects of double reconstructive plate and anatomic locking plate in the treatment of spiral fracture of middle and distal humerus. Methods 44 case of middle and distal spiral humeral fracture were treated with double reconstructive plate (group B, n=25) and anatomic locking plate (group A, n=19), respectively. The operation time, bleeding volume, union time, complications, and function of elbow joint were compared between the two groups. Results All the patients were followed up 11.3 months. The cured rated was 100%. The operation time and bleeding volume were (74.53±9.92) min and (106.84±22.12) ml in group A, and were (92.48±8.93) min and (139.20±32.26) ml in group B, both P < 0.05. There were no statistical differences in the union time and elbow joint function 6 months after the operation between these two groups (both P > 0.05). Conclusions Anatomical locking plate in the treatment of spiral fracture of middle and distal humerus is easy in operation and has low bleeding volume and short operation time, so it is an effective treatment method.
