不同子宫切除术对患者术后性功能及盆底功能的影响比较

摘　　要：目的观察与比较不同子宫切除术对患者术后性功能及盆底功能的影响程度。方法选取2016年10月至2017年3月本溪市中心医院诊治的的子宫切除术90例患者作为研究对象。将其随机分为A组和B组,每组各45例。A组进行腹腔镜筋膜外子宫切除术治疗,B组进行腹腔镜筋膜内子宫切除术治疗。比较两组患者手术前后不同时间的女性性功能指数(BISF-W量表)、盆底功能障碍问卷(PFDI-20问卷)及盆底结构指标。结果手术前两组患者的BISF-W量表、PFDI-20问卷评分及盆底结构指标比较,差异无统计学意义(P>0.05),手术后3个月及6个月B组的BISF-W量表、PFDI-20问卷评分及盆底结构指标显著好于A组,差异具有统计学意义(P<0.05)。结论腹腔镜筋膜内子宫切除术对患者的性功能及盆底功能的不良影响相对更小,更为适用于子宫切除术患者。Objective To observe and compare the influence degree of different hysterectomy for the postoperative sexual function and pelvic floor function of patients. Methods 90 patients with hysterectomy in Benxi Central Hospital from October 2016 to March 2017 were selected as the study object,and they were randomly divided into group A and group B with 45 cases in each group. The group A were treated with laparoscopic extrafascial hysterectomy , the group B were treated with laparoscopic intrafascial hysterectomy. Then the brief index of sexual function for women (BISF-W scale), pelvic floor dysfunction questionnaire ( PFDI-20 questionnaire ) and pelvic floor structure indexes of two groups at different time before and after the operation were compared. Results The BISF-W scale,PFDI-20 questionnaire and pelvic floor structure indexes of two groups before the operation were compared, there were no statistical significances (P > 0. 05 ), the BISF-W scale, PFDI-20 questionnaire and pelvic floor structure indexes of group B at third and sixth month after the operation were all significantly better than those of group A,there were statistical significances(P <0. 05). Conclusions The bad influence of laparoscopic intrafascial hysterectomy for the postoperative sexual function and pelvic floor function of patients are relatively smaller, so it is more suitable to the patients with hysterectomy.
