摘　　要：目的探讨结核感染T细胞斑点试验(T-SPOT.TB)在结核性胸膜炎诊断中的应用价值。方法回顾性分析2017年8月至2018年6月武汉大学人民医院收治的77例疑似结核性胸膜炎患者病例,收集患者结核感染史、卡介苗接种、临床诊断、结核菌素纯蛋白衍生物(PPD)试验及T-SPOT.TB检测结果。分析T-SPOT.TB检测在结核性胸膜炎诊断中的应用价值。结果77例疑似结核性胸膜炎患者分为临床诊断结核性胸膜炎组31例,排除结核性胸膜炎组46例。T-SPOT.TB和PPD检测的灵敏度、特异度、阳性预测值、阴性预测值分别为96.8%、91.3%、88.2%、97.7%和48.4%、76.1%、57.7%、68.6%。T-SPOT.TB检测的灵敏度、特异度均高于PPD,差异有统计学意义(P<0.05)。结论T-SPOT检测可辅助诊断结核性胸膜炎,其诊断的敏感性、特异性均较高。Objective To evaluate the application value of tuberculosis infection of T-cell spot test (T-SPOT. TB) in the diagnosis of tuberculous pleurisy. Methods From August 2017 to June 2018, a total of 77 patients with suspected tuberculous pleurisy in Renmin hospital of Wuhan university were enrolled in this study. Imageology character, clinical manifestation and diagnosis ,PPD and T-SPOT. TB testing result were collected. According to the results of clinical diagnosis, the diagnosis value of T-SPOT. TB was evaluated. Results 31 patients had confirmed tuberculous pleurisy,and the other 46 patients did not have tuberculosis. The sensitivity,specificity ,positive predictive value, and negative predictive value of T-SPOT. TB and PPD test were 96. 8%,91. 3%, 88. 2%,97. 7% and 48. 4%,76. 1%,57. 7%,68. 6% respectively . The sensitivity and specificity of T-SPOT. TB assay were higher than PPD test ,the difference was statistically significant (P<0. 05). Conclusion T-SPOT . TB assay had a higher sensitivity and specificity in the diagnosis of tuberculous pleurisy ,which could provide useful in the diagnosis of tuberculous pleurisy .
