摘 要:目的探讨鞘内注射普瑞巴林对带状疱疹后神经痛(postherpetic neuralgia,PHN)模型大鼠的疗效和脊髓环氧化酶2(cyclo-oxygen-ase2,COX2)表达的影响。方法用SPSS 19.0的随机数字发生器将32只SPF级SD大鼠分为4组(n=8):空白对照组(C组)、PHN组、鞘内注射对照组(PHN-NS组)及鞘内注射药物组(PHN-pr组)。除C组外,其余各组均经腹腔注射0.2μg/g的树脂毒素复制PHN模型。PHN-pr组在疼痛阈值稳定后经鞘内置管泵注600μg/24 h的普瑞巴林,PHN-NS组则仅注射等体积的生理盐水,每天监测大鼠的疼痛阈值变化,连续治疗21 d后采集脊髓匀浆液ELISA法检测IL-6和TNF-α浓度、取脊髓L_(4~6)节段使用免疫组织化学染色及western blot等方法检测COX2表达变化。结果与C组比较,第7天时,PHN模型的机械痛阈值稳定;与PHN组比较,在第22天时,PHN-pr组的机械痛阈值显著增加,差异有统计学意义(P<0.05),而PHN-NS组差异无统计学意义(P>0.05)。与C组比较,PHN组脊髓COX2的蛋白相对表达量、脊髓匀浆液IL-6和TNF-α浓度均显著增加,差异有统计学意义(P<0.05)。与PHN组比较,PHN-pr组脊髓COX2蛋白相对表达量、脊髓匀浆液IL-6和TNF-α浓度显著降低,差异有统计学意义(P<0.05),而PHN-NS组差异无统计学意义(P>0.05)。结论鞘内注射600μg/24 h的普瑞巴林可能通过下调PHN模型大鼠脊髓COX2表达,减少脊髓炎症因子浸润,实现较好的镇痛效果。Objective To investigate the effects of intrathecal injection of pregabalin on the behavioral effects of the postherpetic neuralgia(PHN) model and the expression of COX2 in the spinal cord. Methods Using SPSS19.0 random digital generator, 32 SPF class SD rats were divided into 4 groups(n=8): blank control group(Group C), PHN group(Group PHN), intrathecal injection control group(Group PHN-NS) and intrathecal injection drug group(Group PHN-pr). Except for Group C, the other 3 groups received intraperitoneal injection of resintoxin(0.2 μg/g) to replicate the PHN model. After the pain threshold of the PHN model was stable, intrathecal administration of Pregabalin(600 μg/24 h) was applied in Group PHN-pr, while saline was used as placebo in Group PHN-NS. The pain of the rats was monitored for 21 consecutive days. The levels of IL-6 and TNF-α were detected by ELISA in the spinal cord homogenate, and the expression of COX2 was detected by immunohistochemical staining and western blot. Results Compared with Group C, the mechanical pain threshold of the PHN model was stable on Day 7(P>0.05). Compared with Group PHN, the mechanical pain threshold was significantly increased in Group PHN-pr(P<0.05), but there was no significant difference between the Group PHN-NS and PHN-pr on Day 22(P>0.05). Compared with Group C, the relative expression of COX2 protein of spinal cord, and spinal cord homogenate IL-6 and TNF-α concentrations were significantly increased in Group PHN(P<0.05). Compared with Group PHN, the relative expression of COX2 protein in spinal cord, IL-6 and TNF-α concentrations in spinal cord homogenate were significantly reduced in Group PHN-pr(P<0.05), but there was no significant difference between Group PHN and PHN-NS(P>0.05). Conclusion Intrathecal injection of Pregabalin could down-regulate the expression of COX2 in the spinal cord of PHN model rats, reduce the infiltration of myelitis inflammatory factor and achieve better analgesic effect.
