心电图Sokolow、Cornell指数及两者联用对左心室肥厚的诊断价值

摘　　要：目的评估心电图Sokolow指数、Cornell指数及两者联合用于左心室肥厚的诊断价值。方法选取威海市中心医院2016年3月至2018年3月收治的经心电图检查诊断为左心室高电压的150例患者,回顾性分析其临床资料,全部患者均接受常规心电图检查与超声心动图检查,以超声心动图检查结果作为金标准,评估心电图Sokolow指数、Cornell指数及Sokolow指数+Cornell指数诊断左心室肥厚的准确性、特异度与灵敏度,同时比较三种诊断方法与超声心动图检查的一致性。结果150例左心室高电压患者经超声心动图检查诊断为左心室肥厚73例,左心室肥厚检出率为48.67%;Sokolow指数+Cornell指数诊断左心室肥厚的准确性、特异度、灵敏度均优于单用Sokolow指数或Cornell指数;Sokolow指数、Cornell指数、Sokolow指数+Cornell指数诊断结果与超声心动图Kappa一致性系数分别为0.329、0.426、0.750,Sokolow指数+Cornell指数诊断结果与超声心动图诊断结果的一致性最高。结论心电图Sokolow指数与Cornell指数联合诊断左心室肥厚,可以提高诊断准确性、灵敏度与特异度,与超声心动图检查结果一致性高,临床应用价值高。Objective To evaluate and compare the value of electrocardiogram (ECG) Sokolow index, Cornell index and their combination in the diagnosis of left ventricular hypertrophy. Methods The clinical data of 150 patients with left ventricular high voltage diagnosed by ECG from March 2016 to March 2018 in Weihai Central Hospital were retrospectively analyzed. All patients were examined by routine ECG and echocardiography. The accuracy, specificity and sensitivity of Sokolow index, Cornell index and Sokolow index plus Cornell index in the diagnosis of left ventricular hypertrophy were evaluated with the results of ECG as the gold standard, and the consistencies of the three diagnostic methods with echocardiography were compared. Results Left ventricular hypertrophy was found in 73 cases by echocardiography, among 150 patients with left ventricular high voltage, and the detection rate of left ventricular hypertrophy was 48.67%. The accuracy, specificity and sensitivity of Sokolow index plus Cornell index in diagnosis of left ventricular hypertrophy were better than those of the two simple methods. The consistency of diagnostic results of Sokolow index, Cornell index, Sokolow index plus Cornell index with echocardiogram Kappa were 0.329, 0.426 and 0.750, respectively;and the results of the Sokolow index plus Cornell index had the best consistency with the echocardiogram. Conclusions The combination of Sokolow index and Cornell index of ECG can promote accuracy, specificity and sensitivity of diagnosis of left ventricular hypertrophy, and it has high consistency with echocardiogram and high application value.
