活血通络汤治疗盆腔炎性疾病后遗症47例

摘　　要：目的:观察活血通络汤治疗盆腔炎性疾病后遗症的临床疗效及其对患者炎性因子的影响。方法:将94例盆腔炎性疾病后遗症患者随机分为观察组和对照组,每组各47例。对照组患者给予常规西药治疗,观察组在对照组的基础上加用活血通络汤。观察两组患者的临床疗效,比较两组患者治疗前后中医证候积分变化情况,检测两组患者治疗前后血清炎性因子[肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素-6(interleukin-6,IL-6)、白细胞介素-8(interleukin-8,IL-8)]水平、子宫血流指标[搏动指数(pulsation index,PI)、血管阻力指数(resistance index,RI)、子宫动脉收缩期峰值流速(peak systolic velocity,PSV)]及外周血转化生长因子-β(transforming growth factor-β,TGF-β)水平变化情况。结果:观察组有效率为95. 74%,对照组有效率为80. 85%,两组患者临床疗效比较,差异具有统计学意义(P <0. 05)。两组患者治疗后中医证候积分均低于本组治疗前,且观察组低于对照组,差异具有统计学意义(P <0. 05)。两组患者治疗后血清TNF-α、IL-6、IL-8、PI、RI、TGF-β水平均低于本组治疗前,且观察组低于对照组,差异具有统计学意义(P <0. 05)。两组患者治疗后PSV高于本组治疗前,且观察组高于对照组,差异具有统计学意义(P <0. 05)。结论:活血通络汤治疗盆腔炎性疾病后遗症可有效改善患者机体炎症状态及血液循环,促进炎症吸收。Objective:To observe the influence of Huoxue Tongluo Decoction(HXTLD)on sequela of pelvic inflammatory disease.Methods:Ninety-four patients with sequelae of pelvic inflammatory disease were randomly divided into the observation group and the control group,with 47 cases in each group.The patients in the control group were treated with routine Western medicine,while the ones in the observation group were treated with HXTLD on the basis of the control group.Then the clinical efficacies of the two groups were observed.The two groups’changes of TCM syndrome scores before and after the treatment were compared.The serum inflammatory levels of factors[tumor necrosis factor-alpha(TNF-α),interleukin-6(IL-6),interleukin-8(IL-8),and the changes of uterine blood flow indexes pulsation index(PI),vascular resistance index(RI),peak systolic velocity(PSV)]and peripheral blood transforming growth factor-beta(TGF-beta)levels before and after treatment of the two groups were detected.Results:The effective rate was 95.74%in the observation group and 80.85%in the control group.There was a significant difference between the two groups(P<0.05).After the treatment,the scores of TCM syndromes in both groups were lower than those before the treatment,and the observation group was lower than the control group,the difference was statistically significant(P<0.05).The serum levels of TNF-α,IL-6,IL-8,PI,RI and TGF-beta in the two groups after the treatment were lower than those before the treatment,and the observation group was lower than the control group,and all the differences had statistical significance(P<0.05).The PSV of the two groups after the treatment was higher than that before the treatment,and the observation group was higher than the control group,the difference was statistically significant(P<0.05).Conclusion:HXTLD has a remarkable clinical curative effect on sequela of pelvic inflammatory disease,can effectively improve the inflammatory state of patients and promote the absorption of inflammation.
